Initial all boxes (the IC column on left) directly under the Business Futures, Inc. information. 3. Sign and date the Certification of Independent contraction section. Fill in social security #. This first section should be notarized. (Your bank will do it for nothing). Do not fill in the Certification by Business. 4.
