<p> University of Chicago Office of Intramurals, Recreation and Sport Clubs Ratner Athletics Center 5530 South Ellis Avenue, Room 141 Chicago, IL 60637</p><p>The following are the agreement and condition requirements for all University of Chicago Sport Club instructors, coaches, and guest instructors/coaches in order to provide services to the University of Chicago Sport Club program.</p><p>A supplier, independent contractor or other party may NOT instruct, coach or provide (or receive payment for) a service for a representative of the University of Chicago Sport Club program until all applicable forms below have been completed (and submitted). All instructors and coaches – whether paid or volunteer – are subject to a background check each academic year.</p><p>1. Submit an Instructor-Coach Agreement to be recognized by the University of Chicago Sport Club program as an instructor, coach or guest instructor/coach </p><p>2. Submit detailed resume of experience in related field of instruction/coaching</p><p>3. Submit a minimum of 3 references outlining related experiences (including name, company, address, e-mail, phone number, etc.)</p><p>4. If being paid, complete an Independent Contractor W-9, Independent Contractor Certificate of Compliance and an Independent Contractor Questionnaire (U.S. resident)</p><p>5. If being paid, complete a W-8BEN-I Certification of Foreign Status of Beneficial Owner For the United States Tax Withholding, Request for Alien Information, Certification of Compliance, and Consultant and Independent Contractor Questionnaire (Non-Resident)</p><p>6. Complete an Instructor-Coach Waiver</p><p>7.
