<p> TRINITY LUTHERAN CHURCH PRESCHOOL SCHOLARSHIP APPLICATION INFORMATION</p><p>Eligibility Information: </p><p>1. Applicants must have a child enrolled at Trinity Preschool</p><p>2. Applicants may apply for scholarship no more than one time annually. Awards will be given quarterly for a maximum of 4 months. </p><p>3. Applications must be received no later than the 15th of the month to be eligible for the following quarter. </p><p>4. TLCP is prohibited from discriminating on the basis of race, color, national origin, sex, age, marital status, or disability. </p><p>Eligibility Requirements:</p><p>1. Those receiving county assistance may apply for emergency funds assistance due to: loss of income, large repair bills, or other stated emergency needs. Documentation of need will be needed to apply. Maximum award will be $200.00 and may be used no more than 1 time annually. </p><p>2. Those applying for scholarship and not receiving government assistance will need to complete application and fall within the 150% to 250% of poverty guidelines. </p><p>3. Accounts applying for scholarship may be no more than 1 month delinquent.</p><p>4. Child must be enrolled for 3 or more days per week. </p><p>Documentation of Need: </p><p>1. Applicants will need to submit 2 month’s prior pay stubs for each income earner in the household. </p><p>Conditions of Award:</p><p>1. The award will be a partial scholarship and is not intended to fully fund childcare. </p><p>2. Awards are granted on a per child basis. </p><p>3. There is a 4 month maximum term on awards. </p><p>4. Scholarship will be revoked in the event of fees not paid in time according to tuition contract or in the event of determining that parents have not provided accurate income information. </p>
