Personal Statement Tri-City Hospital Auxiliary 2014-2015 NURSING SCHOLARSHIP APPLICATION DEADLINE: 12 NOON, FEBRUARY 28, 2014</p><p>Last Name ______First ______</p><p>Major: ______School ID Number ______</p><p>To be completed by Scholarship office: Cum Units#______Cum GPA#______</p><p>Tri-City Hospital Auxiliary Scholarship Committee has established the following Eligibility Criteria and Structure as the basis for Nursing School scholarship candidates.</p><p>ELIGIBILITY CRITERIA  MiraCosta College Nursing student  A resident of Carlsbad, Oceanside, San Marcos, or Vista  Minimum 3.0 GPA,  Letter of Recommendation from an Instructor  Include a Personal Essay (see below)</p><p>STRUCTURE OF THE SCHOLARSHIP  Scholarships are for $1,000 per academic year  The scholarship is to be used for tuition, fees, books, supplies, lab fees, uniforms, or computer accessories  The scholarship is not automatically renewable, recipients must reapply each year  Scholarships are not awarded to those working towards a Master’s degree  Recipients will be notified by leter of the scholarship award. Scholarships will be awarded by certificate only. Scholarship funds will be dispersed by the recipient’s school.  Scholarships are granted regardless of gender, race, color, creed or disability  The Auxiliary will be notified by the recipient’s school if the student awarded a scholarship discontinues or fails the nursing program.  Scholarship recipients will be invited to the Auxiliary Scholarship Awards Night on Thursday, May 1, 2014 at Tri-City Hospital</p><p>PERSONAL ESSAY Attach a one-page, typed, essay with your name and student ID# at the top of the page.
