<p> 1642 E 56th STREET SUITE 109 CHICAGO, IL 60637 773-888-1DRS</p><p>Please include the following applicable records when referring a patient for an impairment rating:</p><p> o All treatment records (This includes outside treating physicians- specialists, therapists, etc.)</p><p> o If the patient has a history of work related injury(s) to the area of chief complaint past records will be necessary to calculate apportionment.</p><p> o Radiology reports</p><p> o In cases of joint laxity please note necessary stress measurements (affected vs. unaffected side)</p><p> o In cases of misalignment please note necessary angulation measurements </p><p> o In cases of arthritis please include the cartilage interval along with specific arthritic findings (ie. Osteophytes, cystic changes- one or both sides of the joint, bony collapse, etc.)</p><p> o CT scan reports</p><p> o MRI reports</p><p> o EMG reports</p><p>Including all applicable records indicated above insures the most accurate rating of your patient.
