When planning for your laboratory costs, use the price link within the Stanford billing workbook to gather Stanford Laboratory prices . In addition to the billing workbook, a study protocol form and approved laboratory accession forms are required for Stanford laboratory processing: o the study protocol form is available through the SPCTRM website o send it to Ester Bengil for review and to develop the accession form . If you need additional information about storing and batch processing for specimens, contact Ester directly . Outside Laboratories o Some outside shipments can be made through Stanford Labs; these should be noted on the accession form special directions o Pickup times: FEDEX 3:30pm; DHL 4:30pm; drop off at least 1.5 hours prior to pickup time. o For other outside shipping, contact SPCTRM and other researchers for recommendations . Dry Ice for Shipping o Stanford lab provides dry ice for specimens they ship o SPCTRM managed dry ice distribution that is covered through indirect costs; this is located at the GCRC o You can have dry ice delivered to your office through a variety of local services . Study kits and research participant blood draws at Stanford Laboratory o Research participants should bring their own study kits in order to avoid misplaced study kits at the lab . Required training for handling blood specimens and for shipping blood specimens for Stanford University employees: o Shipping Certification: http://www.stanford.edu/dept/EHS/prod/researchlab/bio/practical.html#danger_go ods To take the Biological Shipping Training Course, log in to the STARS system via http://axess.stanford.edu (click on the "Training" tab and register for and complete the online training class EHS-2700, DOT: Shipping of Dangerous Biological Goods. ** This training will provide the necessary information for you to become certified for Shipping of Dangerous Goods. You can also access a guide published by the University of New Hampshire (click here) for additional reference date. Upon completion of the training, you must take the written exam (click here for Shipping Exam). Passing the exam will allow you to become certified for two years and/or until regulatory changes occur. o Blood Born Pathogens: Also located through AXESS Bloodborne Pathogens (EHS-PROG-1600) .
