(Please note that Salt Lake Community College cannot make an honorarium and/or incidental expense payment to you if you have received such payments from more than four of these organizations within the past six months.)</p><p>Certification</p><p>I certify that the information contained on this form is to the best of my knowledge and belief, true and complete.</p><p>Signature: ______Date: ______</p><p>The American Competitiveness Workforce Improvement Act of 1998 allows payment of honoraria and associated incidental expenses to B-1, B-2, WB and WT visa holders for “usual academic activity”, if paid by a United States institution of higher education, a nonprofit organization affiliated with an institution higher education, or a nonprofit or a governmental research organization. Under the Act, an academic activity may not exceed nine days at a single institution. In addition, such visa holders cannot accept honoraria and/or incident expenses from more than five such institutions or organizations in the previous six-month period. The provision is effective for activities occurring on or after October 21, 1998. CCB 1/2001 Additional information needed for entrance into the United States</p><p>Visitor Documents needed: Official invitation letter from SLCC. The letter must include details of stay, statement that he will receive compensation for “Academic Activity” such as lecture, teaching, or research. Passport</p><p>Entrance into U.S Obtain INS Form I-94 from airline upon arrival. Complete it and present to INS or border official. Present letter from SLCC regarding details of stay and that you will be receiving compensation. The INS or border official should identify I-94 with correct visa status (B-1).</p>
