Our School Was Informed That There Is a Confirmed Case of ______

Letter to School Staff of Possible Exposure to Infectious Disease

Dear Staff:

You may have been exposed to an infectious disease at work. Exposure to infectious diseases in a school setting is not unexpected and somewhat inevitable. In any work setting including schools, there might be individuals who have a higher risk of complications if exposed to certain diseases.

Examples of conditions that might place an individual at greater risk include severe anemia, immunodeficiency, pregnancy or receiving chemotherapy. Pregnant women, in general, are at no higher risk for getting infected than other individuals. However, if a pregnant woman catches certain infections, there is the possibility of in-utero transmission (spread of infection from mother to fetus).

Examples of infections that can be spread in a school setting and might cause complications include varicella (chickenpox), cytomegalovirus, parvovirus B19 (fifth disease), influenza, measles and rubella (German measles).

Our school was informed that there is a confirmed case of ______.

The purpose of this letter is not to determine the extent, if any, of that risk to you but to encourage you to consult with your healthcare provider (HCP), particularly if you may be at higher risk of complications if exposed. Your HCP will assess the risk and make appropriate recommendations.

To keep yourself and our school community healthy and safe, you are encouraged to: . wash your hands with soap and water or use alcohol-based hand gel . not share forks, cups and food with others . get vaccinated as recommended by your healthcare provider . teach students to reduce droplet infections, e.g. by coughing into their elbow

Please do not hesitate to come to me with any questions or concerns.

Thank you,

Signature Title

Acknowledgement of Receipt of Information – Return to your site administrator

Employee Signature

Date

Student, Family and Community Support Department 2017-2018 School Health Manual Student, Family and Community Support Department 2017-2018 School Health Manual
