<p> Chemistry Lab Safety Agreement Form</p><p>Rules of Laboratory Conduct 1. Conduct yourself in a responsible manner at all times when in the lab. 2. Perform only approved and assigned laboratory work and only when your teacher is present. 3. Your concern for safety should begin even before the first activity. Before starting any laboratory activity, always read and think about the details of the lab assignment. Follow all verbal and written instructions carefully. 4. Know the location and use of all safety equipment in your laboratory. These should include he safety shower, eye wash, fire extinguisher, fire blanket, exit, and evacuation routes. 5. Wear a lab apron and safety goggles for all lab work. Wear closed-toe shoes (no sandals or open-toed shoes), and tie back loose hair. 6. Check chemical labels twice to make sure you have the correct substance and the correct concentration of a solution. Some chemical formulas and names may differ by only a letter or a number. 7. All Chemicals in the lab are to be considered dangerous. Do not touch or taste laboratory materials. Do not bring gum, food, or drinks into the lab. 8. If you are instructed to smell something, do so by wafting or fanning some of the vapor toward your nose. Do not place your nose near the opening of the container. 9. Never look directly down into a test tube; do view the contents from the side. Never point the open end of a test tube toward yourself or your neighbor. Always point the mouth of a test tube toward the wall when heating. 10. Always work in a well-ventilated area. Use the fume hood when working with volatile substances or poisonous vapor. Never place your head into the fume hood. 11. In case of a chemical spill on your skin or clothing, rinse the affected area with plenty of water. If the eyes are affected, rinsing with water must begin immediately and continue for at least 10 to 15 minutes. 12. Minor skin burns should be placed under cold, running water. 13. Heated metals and glass remain very hot for a long time. They should be set aside to cool. Never place hot containers on the pan of a balance. 14. Wash bottles are to be used only as intended, e.g., rinsing glassware and equipment, or adding water to a container. 15. Return equipment, chemicals, aprons, and goggles to their designated locations. 16. Report any accident (spill, breakage, etc.) and injury (cut, burn, etc.) to the teacher. 17.
