Your packet can not be processed until you graduate and pass the content exam Reading Specialist #176, the APT PreK-12 #188 and the Illinois Basic Skills or Test of Academic Proficiency exam.</p><p>Name: Date:</p><p>Livetext portfolio sign-in name: Your portfolio should be as complete as possible, including your Final Program Reflection. Prior to filing for certification, you must submit your portfolio for Review with username to readingprogram (no caps, no spaces). After you have shared it, you must e-mail Dr. Sophie Degener at [email protected] to tell him that it is ready for review. Items to be included in the packet:</p><p>Completed Illinois State Application for Teaching Certificate (73-03C). Write “Reading Specialist” on the line in the Special K-12 Certificate box. (DO NOT mail your ROE or ISBE for processing directly. A check is not required. 1. You will pay this electronically to ISBE later). Download the following items from the Illinois State Board of Education ELIS Educator Licensure Information System website: https://www.isbe.net/Pages/Educator-Licensure-Information-System.aspx. Click on Educator Access in the middle of the screen to log in to your ELIS account. </p><p>2. A copy of your current educator Credentials </p><p>3. A copy of your teaching Assignment History indicating evidence of two years of contractual full-time certified teaching experience (not teaching assistants/aide or substitute). </p><p>If your teaching history is not listed in ELIS, provide a letter of verification of employment from the school’s principal or HR department. The letter must be original, on letterhead and include a signature. </p><p>4. A copy of test Score Reports which show your numerical scores for: A. Reading Specialist Content Test #176 B. Advanced Professional Teaching Test #188 C. Illinois State Test of Basic Skills or Test of Academic Proficiency</p><p>Provide the first and last name and the student’s grade level of students tutored during your Practicum (RLR 592A,B) experience.</p><p>Student tutored: Grade level:</p><p>5. The transcript must indicate the degree 6. received and the date the degree was awarded.</p><p>All communication regarding your credentials will be sent through your NLU e-mail account.
