(Prevent Alcohol and Risk- related Trauma in Youth) at the P.A.C. (Performing Arts Center) in Appleton. Since 1998, Theda Clark Medical Center Foundation has supported this educational program, which invites local students to see and hear firsthand what can happen as a result of making poor and risky choices. Tenth grade students are invited to the Fox Cities Performing Arts Center to witness a reenactment of care that is provided to the trauma victim. Students will see the consequences of choices related to risky behaviors, meet individuals whose lives have changed because of their poor choices and hear from individuals who were victims because of poor choices made by others. The goal of the program is to educate people and reduce the possibility of becoming traumatically injured. Wearing a seat belt, not drinking and driving, and taking a few seconds to think about a decision can save lives. Thousands of tenth grade students in the Fox Cities area have attended the program. This year, New Holstein tenth graders have been invited to participate. The show time for our school is Thursday, October 13, from 9:15 a.m. - 11:00 a.m. We plan to leave school at 7:50 a.m., and return around noon. Sophomores will report to their 5th hour class or lunch when we return. The program is free and busing will be provided by Teaming for a Brighter Tomorrow. Faculty chaperones will accompany the students to the Performing Arts Center. If you have any questions or would not like your son or daughter to attend, please contact Doug Olig, Principal, or Ben Zoelle, Counselor, at 898-4256 and return the bottom portion of this letter by September 30. If the bottom portion is not returned, it will be assumed that your student will attend the program. Please fill out and return the Off Campus Release Form on the back of this letter so your son or daughter may attend the program. Your student should return the completed form to the guidance office by September 30. We are excited to have your student see this reenactment. After viewing the program, students are encouraged to discuss the program with parents, teachers, and counselors. Sponsors of the program feel that if this presentation can prevent just one high school student from becoming a trauma victim, it will be worthwhile. Sincerely,

Doug Olig Principal

I prefer that my student ______not attend the P.A.R.T.Y. at the P.A.C. on October 13, 2016. ______Signature of Parent/Guardian

…………………………………………………………………………………………………………………………………………………………………………………… ………………………………………………………………………………… No person shall on the basis of age, race, color, creed, national origin, sex, physical, mental, emotional, learning, or developmental disability, handicapping condition, marital or parental status, ancestry, sexual orientation, arrest record, conviction record, religion, pregnancy, physical condition, membership in the national guard, state defense force or any other reserve component of the military forces of the United States or this state or use or nonuse of lawful products be excluded from participation in, be denied the benefits of, or be subjected to discrimination in any program, activity or employment by the School District of New Holstein as required by Title VI, Title IX, Section 504, and Title II of the ADA.
