Lead Pilot duties and responsibilities are listed in section 3.4 of this manual.</p><p>9.1.4 The procedures and practices listed in this section pertain to satellite operations that fall under part 135 of the Federal Aviation Regulations. </p><p>9.2 SATELLITE BASE LISTING</p><p>9.2.1 Madison 115 East State Capitol Madison, WI 53702 805-351-8307</p><p>9.2.2 Austin 1100 San Jacinto Austin, Texas 78701 909-723-5660</p><p>9.2.3 Fresno 2550 Mariposa Mall #3013 Fresno, CA 93721 949-313-0673</p><p> gwrb COMPANY AVIATION SERVICES, LLC Section 9 GENERAL OPERATIONS MANUAL Page 3 Date 07-01-04 Satellite Base Operations Revision Original</p><p>9.3 LEAD PILOT LISTING</p><p>9.3.1 Madison: Eddie Rickenbacker</p><p>9.3.2 Austin: Jonathan Swift</p><p>9.3.3 Fresno: Henry More Smith</p><p>9.4 BASE RESOURCES</p><p>9.4.1 As per 3.4.9 of this manual, it is the responsibility of the Lead Pilot to ensure the required resources are available.</p><p>9.4.2 Each satellite base will have, at the disposal of the base employees, a copy of:</p><p>(a) The Company Operations Manual.</p><p>(b) The current Operations Memos.</p><p>(c) The current Operations Notices.</p><p>(d) The current FAR’s.</p><p>(e) A collection of pertinent Advisory Circulars.</p><p>9.4.3 Base pilots are encouraged to utilize base resources.</p><p> gwrb COMPANY AVIATION SERVICES, LLC Section 9 GENERAL OPERATIONS MANUAL Page 4 Date 07-01-04 Satellite Base Operations Revision Original</p><p>9.5 RECORD KEEPING</p><p>9.5.1 Each satellite base shall maintain current copies of required records.</p><p>9.5.2 As per 3.4.9 of this manual, it is the responsibility of the Lead Pilot to ensure all required records are maintained.</p><p>9.5.3 Each satellite base will maintain an exact and current copy of the each pilot assigned to said base records which will include:</p><p>(a) Medical Certificate</p><p>(b) Pilot certificate(s)</p><p>(c) Drug testing certificate</p><p>(d) Training records </p><p>(e) Aeronautical Experience</p><p>(f) Aircraft assignment</p><p>(g) Pilots current duties</p><p>(h) Initial and recurrent competency, proficiency and route checks.</p><p>(i) Pilots flight time</p><p>(j) Check pilot authorization, if applicable</p><p>9.5.4 A current list will be maintained of the aircraft used or available for use in operations at said satellite base under part 135 and the operations for which each is equipped. </p><p>9.5.5 Each satellite base must keep the record as stated in 9.4.4 for at least 6 months.</p><p>9.5.6 For multi-engine aircraft, each satellite base is responsible for the preparation and accuracy of a load manifest in duplicate containing information concerning the loading of the aircraft. The manifest must be prepared before each take-off and must include:</p><p>(a) The number of passengers</p><p>(b) The total weight of the loaded aircraft.</p><p>(c) The maximum allowable takeoff weight for that flight</p><p>(d) The center of gravity limits.</p><p> gwrb COMPANY AVIATION SERVICES, LLC Section 9 GENERAL OPERATIONS MANUAL Page 5 Date 07-01-04 Satellite Base Operations Revision Original</p><p>9.5 RECORD KEEPING, Continued…</p><p>9.5.7 The satellite base shall keep copies of completed load manifests as per 9.4.6 of this section for at least 30 days.</p><p>9.5.8 An aircraft maintenance log will be carried on board each aircraft for recording and deferring mechanical irregularities and their correction.</p><p>9.5.9 Copies of all maintenance records for aircraft operating at said base will be maintained.</p><p>9.5.10 A Maintenance Tracking Sheet shall be completed each weekday and a copy transmitted to The Pagosa office.</p><p>9.5.11 Each employee of a satellite base that is on salary shall complete his or hers respective duty time sheet and transmit a copy to The Pagosa office.</p><p>9.6 FLIGHT FOLLOWING</p><p>9.6.1 Flight location procedures will be established for each satellite base as per section 16 of this manual.</p><p>9.6.2 As per 3.4.9 (m) of this manual, it shall be the responsibility of the Lead Pilot to manage the flight following program.</p><p> gwrb COMPANY AVIATION SERVICES, LLC Section 9 GENERAL OPERATIONS MANUAL Page 6 Date 07-01-04 Satellite Base Operations Revision Original</p><p>Page Left Blank Intentionally</p><p> gwrb</p>
