DIACONAL PROBATIONER FORM A (2018)

Summary of Assessments

Notes

This report is designed to summarise the evidence presented in individual reports to present a snapshot of the probationer’s development so far and their competencies at this point, in a way that will assist their future development. It is not concerned only with assessing whether they have reached a required standard. At the same time the evidence summarised here must form the basis for the Committee’s judgement as to their fitness either to continue on probation or to be ordained. It is important that the Committee is aware that all deacons are part of a religious order as well as an order of ministry and that therefore the Committee is looking for evidence of an understanding of and participation in the life of the Methodist Diaconal Order and a living out of the Rule of Life. Each area of the report should make use of evidence drawn from individual reports (eg circuit report, report from the Warden of the Order) as indicated. Note should be made of, for example: the probationer’s strengths and weaknesses; hopes and concerns of and about the probationer; insights into ways of building on strengths and dealing with weaknesses. For more detail under each heading please refer to the ‘outcomes for probation’ in the Handbook for Ministerial Probation. The Ministerial Candidates and Probationers Oversight Committee is aware that splitting the individual reports in this way makes more work for the DPS, but believes that the result will be a more comprehensive and informative report. This report is confidential to the District Probationers’ Committee, MCPOC, the Warden, the probationer and his/her Supervisor. In the event of a probationer moving districts, this report will be shared with the probationer’s new Superintendent, District Probationers’ Secretary and District Chair. This form should be returned by the District Probationers’ Secretary as a Microsoft Word document attached to an email to [email protected] by 9.00am on Monday 9 April 2018. Many thanks. Copies of all the forms (A-F) should also be sent to the MDO Office [email protected] for their archives.

Page 1 of 8

The Connexional Team Methodist Church House, 25 Marylebone Road, London NW1 5JR | Helpdesk: 020 7486 5502 www.methodist.org.uk Registered charity number 1132208 Diaconal Probationer Form A – 2018 Section 1

Basic information

1 | Probationer name

<insert>

2 | Circuit name and number

<insert>

3 | Year on probation

<insert> of <insert>

Section 2

Vocation: call and commitment (including participation in the life of the Diaconal Order and living the Rule of Life)

As evidenced in:

4 | Summary of self-assessment

<insert>

5 | Circuit report

<insert>

6 | Interviews with the DPC

<insert>

7 | Report from the Warden of the Order

<insert>

Section 3

Being in relationship

As evidenced in:

Page 2 of 8 Diaconal Probationer Form A – 2018 8 | Summary of self-assessment

<insert>

9 | Circuit report

<insert>

10 | Interviews with the DPC

<insert>

11 | Report from the Warden of the Order

<insert>

12 | Report from Worship Development Group

<insert>

13 | Other reports

<insert>

Section 4

The Church’s ministry in God’s world

As evidenced in:

14 | Circuit report

<insert>

15 | Report from Worship Development Group

<insert>

16 | Report from the Warden of the Order

<insert>

Page 3 of 8 Diaconal Probationer Form A – 2018 17 | Orientation project (first year) or Gospel in Context project (second or final year)

<insert>

18 | Interviews with the DPC

<insert>

19 | Other reports

<insert>

Section 5

Leadership and collaboration

As evidenced in:

20 | Summary of self-assessment

<insert>

21 | Circuit report

<insert>

22 | Report from Worship Development Group

<insert>

23 | Interviews with the DPC

<insert>

24 | Report from the Warden of the Order

<insert>

25 | Other reports

<insert>

Page 4 of 8 Diaconal Probationer Form A – 2018

Section 6

Learning and understanding

As evidenced in:

26 | Report on studies (Form E)

<insert>

27 | Summary of self-assessment

<insert>

28 | Circuit report

<insert>

29 | Orientation project (first year) or Gospel in Context project (second or final year)

<insert>

30 | Interviews with DPC including evidence of engagement with theology and culture (booklist or other evidence)

<insert>

31 | Report from the Warden of the Order

<insert>

Section 7

Communication

As evidenced in:

32 | Circuit report

<insert>

Page 5 of 8 Diaconal Probationer Form A – 2018

33 | Report from Worship Development Group

<insert>

34 | Report from the Warden of the Order

<insert>

35 | Orientation project (first year) or Gospel in Context project (second or final year)

<insert>

36 | Interviews with the DPC

<insert>

37 | Other reports

<insert>

Section 8

Participation in the life of the Religious Order

As evidenced in

38 | Report from the Warden

<insert>

39 | Summary of self assessment

<insert>

40 | Other reports

<insert>

Page 6 of 8 Diaconal Probationer Form A – 2018 Section 9

District Probationers’ Committee Report and Recommendations

The Committee is asked to assess, based on the evidence indicated above, the general fitness of the probationer as someone who is: Adequately formed in the being and role of a Methodist deacon (including participating in the life of the Religious Order and living out the Rule of Life in a way that is appropriate to their circumstances and understanding) Appropriately able to act as a representative person Adequately able to perform the tasks of a deacon

Please note that it is essential that the comments make clear what level of competence you judge the probationer to be at, and on what evidence that judgement is based.
