<p>Fusion (JH) Parent Survey</p><p>Dear Parents, Thank you so much for your support of the Junior High ministry at Lake Bible church (Fusion). We the staff love spending time with your children and having the privilege of seeing them learn and grow in their relationship with Jesus Christ. As a parent, you have a tremendous amount of influence and control over what your children participate in. There are many other activities they could be participating in, so we just want to thank you for valuing this ministry. That said, we also understand that there may be certain things you are dissatisfied with or confused about in regards to this ministry. With this survey, we wanted to give you a chance to express any questions or concerns you have with the youth group. We would love to hear any feedback you have. Feel free to answer all or some of the questions and return them to Bryan in any format (handwritten, typed, emailed, etc.). Bryan’s email address is listed below, or you can drop off a copy to the church office. Bryan is the only person who will be reading these, and would prefer to know who is responding so he can follow up with you, but you are welcome to fill it out anonymously if that is more comfortable for you.
