Resume listing any work, education, publications, teaching and research experience. 2. This department application (Upload it to the Supplementary Information section in the ApplyYourself online application.) 3. Three Recommendations (Forms are located in the Recommendations section of the online application. Recommenders complete them online.) 4. GRE General Test scores (Scores may be sent electronically by GRE.) 5. TOEFL scores (International applicants only. TOEFL may send scores electronically to the University.) 6. Upload your transcripts directly to the ApplyYourself online application; click here for more application instructions. Mailing paper copies of your transcripts to the department can significantly delay your application processing.</p><p>PREREQUISITE COURSEWORK Instructions: List prerequisite coursework below; all courses must be completed or in progress to apply REQUIRED DOCUMENTATION (for certain courses) Applicants may need to provide documentation for each prerequisite course that is not listed under a similar title on their transcripts (such as a calculus course that is listed as “Mathematics” on the transcript, or a course that is titled “Chemistry” that included both general and organic chemistry).
