<p>KC24A Foster carer agreement for emergency & immediate placements (Fostering Services Regulations 2011, Regulation 24) (Copy to be provided to foster carer) Children & Young People’s Social Care</p><p>Name of carer(s): Name of child(ren): Relationship to child(ren): Placement start date: </p><p>I hereby declare that I am related to or friends of the above named child(ren), and, as foster carer(s) of the(se) child(ren), I agree to: - care for the child(ren) as if he/she is a member of my family - allow any person authorised by Leeds City Council Children & Young People's Social Care to visit my/our home at any time - allow the child(ren) to be removed at any time by Leeds City Council Children & Young People's Social Care if the continuation of the placement is deemed to be detrimental to the welfare of the child(ren) - keep any information given to me about the child(ren), or his/her/their family or other person, completely confidential, and not share such information unless permission has been granted by Leeds City Council Children & Young People's Social Care - allow the child(ren) to have contact in accordance with the terms of any court order relating to contact or any other arrangements made or agreed by Leeds City Council Children & Young People's Social Care - ensure that the child(ren) receive necessary medical attention when required in order to maintain his/her/their health and well-being - notify Leeds City Council Children & Young People's Social Care if the child(ren) should go missing - notify Leeds City Council Children & Young People's Social Care of any significant events or information concering the child(ren) I agree not to: - use any form of corporal punishment (e.g. smacking, slapping, hitting, pinching) on the child(ren) - use any means of physical restraint unless absolutely necessary for the prevention of injury to the child(ren) or other persons or serious damage to property</p><p>I understand that this arrangement is a temporary one, and in order to care for a child under the Fostering Services Regulations 2011, a full assessment of my family will be required.</p><p>1st applicant 2nd applicant Signed Date </p>
