<p>Order Form Instructions</p><p>1. If you use MS Word 2007 and you see a Security Warning above the form which says that macros have been disabled do the following: a. Click on the box titled Options b. Check the button which says Enable This Content c. Click OK 2. Complete the form by entering your desired quantities of each item. Remember the following: a. Please enter your identifying information in the boxes on the first line of the form. b. You must enter in multiples of 5 servings (i.e., 5, 10,15) c. You may mix and match your order by selecting only the items you wish. You don’t need to order the suggested vegetables and fruits which correspond to the entrée listed for a given day. d. The total quantity of fruits and of vegetables must equal the total quantity of the entrees unless you have included an entrée in your selection which doesn’t require a vegetable. e. Follow the instructions in the box on the lower right of the form. Your total order must be at least $125 w/o delivery or tax. No tax applies in Minnesota. Be sure to TAB or Click into the Calc box on the last line and then hit TAB again. This will launch an error check of your order. f. Save a copy of the completed order on your computer. g. E-mail your order to [email protected] . h. Go to www.thepreschoolpantry.com and click on the Family Child Care box. Select Contact Us on the top menu. Click on TO MAKE A PAYMENT CLICK HERE and complete the transaction. Your payment must be for the full amount of the order. If you are responding to a $ off promotion your account will be credited after eligibility has been verified and your order has been shipped.
