<p> JOB DESCRIPTION</p><p>Job Details</p><p>Job Title: Bank Occupational Therapist</p><p>Band: 5</p><p>Directorate: Clinical Directorate</p><p>Reporting to: Therapies Team Manager</p><p>Location(s): The primary location is Dorothy House Hospice Winsley, as well as visiting patients in the community and other Dorothy House Venues.</p><p>Dimensions</p><p>Team: The Therapy team is a multi-disciplinary team that comprises of Occupational Therapy, Physiotherapy, Complementary Therapy, Creative Arts and Dietetics.</p><p>Patients: To work with inpatients, day patients and outpatients as part of the Therapies team</p><p>Sites: Inpatient Unit, Day Patient Service, Community and any other Dorothy House Venues </p><p>Job Summary/Main Purpose</p><p> To work as part of the Therapies Team to provide an Occupational Therapy service for the assessment, treatment and on-going management of Dorothy House patients, working both with the patient, their families and carers in liaison with the Multi-Disciplinary Teams to achieve agreed goals.  Provide cover for current staff holidays, sickness, study/teaching leave or periods of heavy clinical pressure, on the IPU/DPU or in the community.</p><p>Principal Duties and Responsibilities</p><p>1. Clinical  To assess patient's Occupational Therapy needs in close liaison with other professionals, carers and families.  To attend report sessions in DPU and contribute to the multi-disciplinary approach to care.  To be available as a resource to DPU patients and facilitate appropriate groups sessions.  To formulate a treatment plan, implement the plan, review and up-date as necessary  To attend multi-disciplinary meetings for the IPU and support other team members in delivering an Occupational Therapy service to patients on IPU, with follow up in the community if necessary  To be available for Occupational Therapy community visits for patients on DH caseload under the guidance and supervision of a Senior Therapist  To maintain accurate clinical records on System 1 2. Management</p><p> To be an active member of the Therapies Team to ensure co-ordination of patient care.  To liaise closely with other members of the Multi-disciplinary Team and, when appropriate, Community Teams (including Social Care, Health and Housing Associations)  To work with Team manager in supervising and supporting junior members of staff and volunteers. </p><p>3. Educational</p><p> To keep up to date with the current research and practice in Occupational Therapy with specific application to palliative care.  To participate in appropriate in-house training.</p><p>4. Professional  To work within the Professional Code of Conduct for State Registered Occupational Therapists.  To participate in clinical supervision and reflective practice.  To work within the policies and procedures of the Hospice.  To participate in annual Performance Reviews.  To take responsibility for own personal development, identifying training needs with line manager.</p><p>Organisational Chart See attached.</p><p>Contacts  Members of the DH multi-professional team, Primary Health Care and acute sector.  Patients, relatives and carers.</p><p>Special Note This job description does not form a part of the contract of employment but indicates how that contract should be performed. It applies to the Hospice premises and grounds at Winsley, all Dorothy House shops and when staff are on duty in patients’ homes.</p><p>Confidentiality All of the work relating to patients is of a confidential nature and information gained must not be communicated to other persons except in the course of duty.</p><p>Health and Safety at Work Act It is the responsibility of all employees to ensure that the requirements of the Health and Safety at Work Act are complied with; safe working practices are adhered to and those hazards are observed and reported to the appropriate office.</p><p>2 PERSON SPECIFICATION</p><p>Band 5 Occupational Therapist</p><p>Essential Desirable  Degree in Occupational Therapy   Current Registration with HCPC   Post Graduate Training in Palliative Care   Minimum of 2 years post graduate clinical experience   Effective organisational and time management skills   Experience of working in partnership across organisational  boundaries  Demonstrates a knowledge of evidence based practice and its  application in Occupational Therapy  Experience of working with people in Palliative Care </p><p> Demonstrates an ability to contribute effectively within the  Multi-disciplinary Team  Excellent communication, networking and interpersonal skills   Demonstrates an awareness of Health and Safety  requirements  Demonstrates sensitivity and listening skills   Experienced and ability to demonstrate skills in Multi  Disciplinary Working  Experience of teaching/education   Commitment to ongoing professional development   Car Driver </p><p>3 4</p>
