Dear (ENTER RESTAURANT MANAGER’S NAME),

On the weekend of September 29th, college coaches and high school coaches around the country will be united in fighting Duchenne muscular dystrophy (Duchenne), the number one genetic killer of childhood. They will be participating in COACH TO CURE MD, a program of Parent Project Muscular Dystrophy – the largest national charity dedicated exclusively to Duchenne research.

When you turn on any televised college football game on September 29th, you will see coaches wearing Coach to Cure patches. What started as a couple of hundred participating schools, has turned into a national awareness campaign that in-stadium audiences and at-home viewers are taking notice of. Last year, more than 10,000 college coaches at over 500 different institutions participated in Coach to Cure. Your restaurant could be part of this unique and important team!

Duchenne muscular dystrophy primarily affects boys and is a genetic condition that can happen in any family. It slowly destroys a boy’s muscle strength, their ability to walk, their ability to breathe. Boys with Duchenne lose their battle in their early 20’s. There are limited treatments and no cure. But there is HOPE. And with money for research a cure can be found.

Across the country there will be many local events in conjunction with Coach to Cure MD. The goal of Coach To Cure MD is to raise awareness and funds for research for Duchenne.

We are requesting your participation in Coach to Cure MD this year. We are asking for your restaurant’s support in the following way(s):

 To allow us to host an event at your location. We will invite people to come to your establishment on September 29th, 2012.

 To donate a portion of your food sales to Coach to Cure MD that day

 To allow us to publicize Coach To Cure in your restaurant

We will also publicize the event, specifically letting local media venues know of the event. We want to let the community know that you care about saving boys with Duchenne.

For more information on Coach to Cure MD, visit our website: www.coachtocuremd.org.

Thank you for your time and consideration, About Coach to Cure MD (CTC)

Coach to Cure MD is a partnership between the American Football Coaches Association (AFCA), a professional organization for over 10,000 college football coaches and staff, and Parent Project Muscular Dystrophy (PPMD), the largest national charity devoted exclusively to Duchenne muscular dystrophy. In 2008 the AFCA adopted PPMD’s Coach to Cure MD program as one of their charity efforts. One reason the AFCA was drawn to Coach to Cure MD was because of the unique parallels between Duchenne, a disorder which robs young men of precious muscle strength and college football, a game where young men are at the peak of their muscle strength.

The goals of Coach to Cure MD are simple: raise national awareness of the disorder and raise money to fund research for a cure. Our approach to reaching these goals is also simple. One football Saturday of each season (this year it will be on September 29, 2012) AFCA coaches nationwide agree to promote Coach To Cure MD. By wearing armbands, mentioning Coach to Cure MD during on and off-field interviews, and in some instances doing even more extensive media relations around the date, coaches are rolling up their sleeves and proudly getting involved.

About American Football Coaches Association (AFCA)

The AFCA is the professional association that represents more than 90 percent of head coaches at American colleges and universities. Founded in 1921, the AFCA has promoted the improvement of the coaching profession and has highlighted the good work of coaches on and off the field.

About Parent Project Muscular Dystrophy (PPMD)

Parent Project Muscular Dystrophy (PPMD) is the largest nonprofit organization in the United States focused entirely on Duchenne muscular dystrophy.

Since 1994, PPMD has improved the treatment, quality of life, and long-term outlook for all individuals affected by Duchenne through research, advocacy, education, and compassion. Because of our efforts, families affected by Duchenne have better access to state-of-the-art care information, research is moving forward at an accelerated pace, and legislation now exists funding Duchenne research and outreach programs.

The name of the organization reflects our grassroots origins, parent-led focus, and passion, and Parent Project Muscular Dystrophy is now a name recognized around the world as the leader in the Duchenne community. We hold the highest ethical standards and consistently receive high marks from watchdog organizations.

We take a comprehensive approach in the fight against Duchenne—funding research, raising awareness, promoting advocacy, connecting the community, and broadening treatment options. Only this comprehensive approach will lead to the day that 100% of those diagnosed can turn to a treatment that will lead to the end of Duchenne muscular dystrophy.

Learn more about Duchenne by visiting our website at ParentProjectMD.org. SAMPLE STORY

Our Story of Duchenne

In September of 2002, we brought home a beautiful baby boy from the hospital who from all accounts looked healthy and perfect. Little did we know that he was already fighting for his life and that his little body was beginning to build up scar tissue.

When he was three we noticed that our son Joel was not jumping, running like the rest of the kids. We ended up going through tests and he was diagnosed with Duchenne muscular dystrophy.

While is it a genetic condition, Duchenne can happen to any family. It weakens boys’ muscles and by the age of 10-12 they lose their ability to walk, and they often lose their battle in their late teens or early twenties. There is no cure and there are limited treatments. But there is hope.

There are several promising research studies in the works to save this generation of boys. But what we need is money and time -- both of which are so hard to come by.

Coach to Cure MD is about letting people across the nation know about this lethal disease and about raising critical money for research.

Our son is in the fight of his life, and with your help we can win it. We can save this generation of boys with Duchenne.

Please help us by being part of the hope.

Ending Duchenne Together,

Rachel Poysky Schools that Participated in 2011:

A - M N - Z

Adams State College NC State University

Air Force Academy NC Wesleyan College

Alcorn State University New Mexico State University

Alfred University Nicholls State University

Allegheny College Nichols College

Alma College, Michigan Norfolk State University

Anderson University North Central College

Angelo State University North Dakota State University

Anna Maria College North Greenville University

Appalachian State University North Park University

Apprentice School Northeastern State University

Arizona State University Northern Arizona University

Arkansas State University Northern Illinois University

Arkansas Tech University Northern Michigan University

Army Northern State University

Ashland University Northwest Missouri State University

Auburn University Northwestern State University

Augustana College (SD) Northwestern University

Austin College Northwood University

Austin Peay University Norwich University

Averett University Notre Dame

Azusa Pacific University Oberlin College

Bacone College Ohio University

Bates College Ohio Dominican University

Baylor University Ohio Northern University

Becker College Ohio State University

Benedict College Ohio Wesleyan University

Benedictine University Oklahoma State University

Bethany College Old Dominion University

Bethel University Oregon State University Birmingham Southern College Otterbein University

Bluffton University Ouachita Baptist University

Boise State University Pacific Lutheran

Boston College Penn State

Bowdoin College Pittsburg State University

Bowie State University Pomona-Pitzer College

Bowling Green State University Portland State University

Brigham Young University Presbyterian College

Brown University Princeton University

Bryant University Purdue University

Buena Vista University Rensselaer Polytechnic Institute

Buffalo State College Rice University

California University of PA Robert Morris University

Campbell University Rose-Hulman Institute of Technology

Campbellsville University Rowan University

Capital University Rutgers University

Carnegie Mellon University Sacramento State University

Carroll University Saginaw Valley State

Carson-Newman College Salisbury University

Castleton State College Salve Regina University

Catholic University Sam Houston State

Central Connecticut State University Samford University

Central Methodist University San Diego State University

Central Michigan University San Jose State University

Central State University Seton Hill University

Centre College Shenandoah University

Chadron State Shepherd University

Chapman University Shippensburg University

Charleston Southern University Shorter University

Cheyney University Simpson College

Chowan University Slippery Rock University

Christopher Newport University South Dakota Mines Clemson University South Dakota School of Mines

Coe College South Dakota State University

Colby College Southeastern Louisana University

Colgate University Southeastern Oklahoma State University

College of Mount Saint Joseph Southern Arkansas University

Colorado School of Mines Southern Connecticut State University

Colorado State University Southern Methodist University

Colorado State University-Pueblo Southern Mississippi

Columbia University Southern Nazarene University

Concordia College Southern Utah

Copiah-Lincoln Community College Southwest Baptist University

Cornell College Southwest Minnesota State University

Cornell University Southwest Mississippi Community College

Curry College Southwestern Assemblies of God University

Dartmouth University Springfield College

Defiance College St. Augustine's College

Delaware Valley College St. John Fisher College

Delta State University St. Xavier University

Dickinson College Stanford University

Drake University Stephen F.
