<p>Working with Health IT Systems: HIT Facilitated Error—Cause and Effect </p><p>Application Activities</p><p>In this assignment, you will interact with the CPRS system as a user would. The focus of this unit is not on knowledge of clinical nuances, since not all students will have clinical experience. There are no orientation videos associated with this unit; skills built in prior units will be called upon to complete this exercise. </p><p>A patient (Sixtyfive, Patient) came in for a visit earlier in the day. Due to extreme business and an emergency in the clinic at the time of this patient’s visit, you (as DOCTOR 1) did not have time to complete the documentation in the CPRS. At 5:30, after the clinic has closed, you will spend some time “catching up” with documentation of patient visits earlier in the day. </p><p> Capture your work for submission to your instructor. Options for doing this may include using the print screen function (with a cut and paste to a document file), or certain software applications such as Screen Print & Capture 32, SnagIT, CaptureWiz, etc. Many actions have a “PREVIEW” button which an optimal place to do a cut and paste of your work. Check with your instructor for submission requirements. </p><p>STEPS: 1. From within the VistA system, select Sixtyfive, Patient and open the record. 2. Click on “Visit Not Selected” in the top tool bar in CPRS, and choose “New Visit.” 3. Chose PRIMARY CARE as the visit location with the date of NOW. 4. Click on the problem tab to record the new problem you discovered in Sixtyfive, Patient earlier today. Choose NEW PROBLEM. 5. The problem that you need to record (Prolapse of Uterus) is not displayed, so choose “Other Problem.” 6. In the new dialog box that appears, search for PROLAPSE OF UTERUS. Select this new medical problem from the search list and click OK. 7. In the ADD PROBLEM box, the problem should be active and acute. The date of onset can be any date within one week prior to the date of the visit. 8. Order a new consult. Choose Gynecology. The reason for request is “uterine prolapsed.” Accept the order and then select QUIT. 9. Click on orders tab to see your new order. Sign the order by right clicking on it and choosing “Sign” from the drop-down box. 10.Now that you have ordered the consult, you can complete your documentation note. Choose “Note” and then “New Note.” 11. From the New Progress Note Dialog pop-up box, choose “GYNECOLOGICAL” and click OK. A “new note in progress” should appear in the Note box to the left. 12.As the clinician, you choose “Template,” because you know that the exam that you are going to perform on Sixtyfive, Patient has a template that will guide your exam and make the documentation easier. 13.Choose “Shared Template,” and then choose “NATIONAL TEMPLATES” and then “GYNECOLOGY.” 14.Your goal is to record the data from your prior visit today with Sixtyfive, Patient. Double click “gynecology” to display the template. 15.Record the data of the gynecology assessment as such: BREAST EXAM – Normal. PELVIC EXAM, all NORMAL except CERVIX which is abnormal. Record the words “prolapsed” in the note field of the gynecology template as part of documenting in the record the findings that led you to choose abnormal in the first place. 16.Scroll to the bottom and click “PREVIEW.” If you need to make corrections, do so. 17.Click OK at the bottom of the template to transfer the note from the template into the patient’s chart. The results of the gynecology assessment will appear in the Note window to the right. 18.Right click in the body of the note that you just transferred from the template. Chose “Sign Note Now.” Sign the note when prompted, or choose “ACTION” from the tool bar to sign the note. The electronic signature is the same as your login ID (i.e. DOCTOR1 or DOCTOR2, or …) 19.Now you need to run a report to send to Sixtyfive Patient’s consultant. Choose the REPORTS tab. 20.In reports, select ADHOC. In the ADHOC Health Summary Dialog that emerges, at the bottom near the left, choose LOOKUP BY: “abbreviation.” The list in the selection box will resort by abbreviations. 21.From that list, choose BDEM (brief demographics); PLL (All problems); BLO (brief lab orders); and SPN (for selected progress notes), remembering to shift each item to the component selection window to the right. 22. In the SPN file selection box that pops up as you shift SPN to the right, choose “C&P Gynecological Conditions & Disorders.” 23.Choose OK to run the report. If so, return to CPRS and conduct an investigation of how these errors may have been facilitated by HIT. Describe the error path in brief detail. Identify several ways that these HIT errors could be prevented with better HIT design and describe them in your write-up.
