<p>Care Coordination Communiqué Blue Plus Government Programs</p><p>Clarification of Health Risk Assessment Audit Process</p><p>August 24, 2016</p><p>Dear Care Coordination Partners,</p><p>Earlier this month, we notified you of our updated HRA Audit within Bridgeview. Some of you have already submitted documentation as part of this audit, while others may not have been identified this month. We thank those of you who have provided documentation to us thus far!</p><p>Just a few reminders on this new process. </p><p>1. When you click on Send Attachment in Bridgeview, your email system will open a new email for you to attach the appropriate documentation. Attach the requested documentation and hit send. Please do NOT change the subject line or the “TO” address field on the email, as these have been prepopulated with required information. When these fields are altered, the emails do not process through our automated system.</p><p>2. You have up to 7 days to submit the requested documentation from the original date of request. If the documentation isn’t provided within that timeframe, a reminder email will be sent every 7 days until all requested documentation is received.</p><p>3. Our automated system can only accept one attachment per email. If you are providing more than one document per member (per email), you must combine them before attaching them to the email. For example, if you are supplying contact notes and an Unable to Contact Letter, please combine them as a PDF, attach to the email and then click send. There are many ways to combine multiple documents, however you choose to complete this process, attach the combined document to the email.
