<p> 2013 LIABILITY RELEASE FORM</p><p>I/We, the parents/guardians of ______assume any and all risks and hazards of the conduct of the Junior Golf Camp program at Trophy Lake Golf & Casting. We acknowledge that the game of golf is not without risks, despite safety precautions. I/We further release, discharge and promise not to sue Trophy Lake Golf LLC. and any/all staff personnel of Trophy Lake Golf LLC for any loss, damage or injury (including death) or liability as regards to injuries suffered by the above mentioned person in connection with his/her participation in the Junior Golf Camp program, unless caused by the sole and gross negligence of Trophy Lake Golf LLC, or their staff personnel. </p><p>BY SIGNING THIS AGREEMENT, YOU ARE GIVING UP CERTAIN LEGAL RIGHTS, INCLUDING THAT RIGHT TO RECOVER DAMAGES IN CASE OF INJURY, DEATH OR PROPERTY DAMAGE.
