Box 149347 Austin, Texas 78714-9347 1-888-963-7111 KIRK COLE TTY: 1-800-735-2989 INTERIM COMMISSIONER www.dshs.state.tx.us</p><p>The International Classification of Diseases, Tenth Revision, Clinical Modification (ICD- 10-CM) and ICD-10-PCS (inpatient procedure code) code sets will replace ICD-9-CM codes that are used to report medical diagnoses and inpatient procedures. The Centers for Medicare & Medicaid Services (CMS) effective date for ICD-10 code sets is October 1, 2015. </p><p>ICD-10 implementation will affect diagnosis and inpatient procedure coding for all entities that are covered by the Health Insurance Portability and Accountability Act (HIPAA). Health-care providers, payers, clearinghouses, and billing services must be prepared to comply with the ICD-10 implementation.</p><p>Beginning October 1, 2015, the Hemophilia Assistance Program (HAP) will require providers to document the medical diagnosis codes and descriptors from the ICD-10- CM when preparing eligibility documentation on behalf of HAP clients.</p><p>ICD-9 ICD-10-CM 286.0 - Hemophilia A D66 - Hemophilia A 286.1 - Hemophilia B D67 - Hemophilia B 286.2 - Hemophilia C D68.1 - Hemophilia C Additional information is available on the CMS website at: www.cms.gov/Medicare/Coding/ICD10. </p><p>An Equal Opportunity Employer and Provider</p>
