Robert Wood Johnson Foundation The San Francisco Partnership for Community-Based Care & Support … is our local initiative. Purpose: To improve access to quality services for older adults and adults with disabilities 11/17/2006 Where we started • Increasing numbers of older adults and adults with disabilities • Limited collaboration between service providers • Lack of parity for disability services • No single authority to oversee improvements in community-based long term care. Optimize the use of available resources for all clients who need care coordination.
