Netbook/Laptop Contract and Consent Form Troy Christian Schools (TCS) is pleased to be able to provide students with a netbook/laptop computer to enhance, enrich, and facilitate learning and communication. The use of a TCS netbook/ laptop is a privilege extended to students for use inside and outside the school facility. The school’s netbook/laptops are provided for use as a productivity tool for school-related purposes, curriculum enhancement, and communications. All netbooks/laptops and related equipment and accessories are TCS property and are provided to the students for a period of time as deemed appropriate by the school’s administration. With this privilege comes responsibility and as a condition of use, students and parents will sign this Netbook/Laptop Contract and Consent form, and comply with and agree to all of the following: Student Parent Initials Initials 1. I have read and agree to abide by the Troy Christian Schools, Inc. Acceptable Use Policy (AUP) for Technology. 2. I have read the TCS Netbook Laptop Handbook, and I understand and agree to abide by its contents. 3. I understand that I am responsible for backing up my own files and that it is important to store files in at least two separate locations. TCS will not be responsible for personal data stored on the netbook/laptop. 4. I understand that during school hours students have monitored access (using electronic filtering and direct supervision) to the Internet and that outside of the school network. I also understand that filtering technologies are never 100% effective, and I agree not to hold TCS liable for material accessed that is offensive as a result of use of TCS technology resources. 5. I have read and agree to abide by Google Apps Responsible Use Guidelines. 6. I agree that I will use the TCS netbook/laptop primarily for school related purposes. 7. I understand that my family is responsible for the cost of replacement should my netbook/laptop and/or its accessories become lost or stolen. I will provide a police report in the case of theft. 8. I understand that my family is financially responsible for the cost of any damage that occurs. 9. I will report problems with my netbook/laptop to a member of the technology support staff in a timely manner. 10. I will not duplicate nor distribute copyrighted materials other than a back-up copy of those items I legally own.
