OHSU School of Nursing 7-Year Time Limit Waiver Request Form for Science Coursework Applicants to the 3 Year Bachelor of Science, Accelerated BS, and Accelerated BS to DNP/PhD programs can use this form to request a waiver on the science prerequisite courses that were completed beyond the 7-year time limit. These courses include: Human Anatomy and Physiology, Nutrition, Introduction to Genetics, Microbiology, and *Statistics.* Please note: Genetics and statistics are not prerequisites for the 3 Year Bachelor’s program. *If you are applying to the Accelerated BS to DNP/ PhD programs, you cannot use this waiver to waive statistics courses taken beyond the 7-year time limit. You must fill out a separate 5-Year Time Limit statistics waiver form.* Note: This waiver does not guarantee that your courses will be waived. After review, you will be notified of whether or not your course(s) have been waived. If you waiver is denied, you will need to come up with a plan to take the indicated course(s) by the time the program starts, if admitted.
