Integrated Therapy and Care Coordination Service (ITACC) Integrated Therapy and Care Co-ordination (ITACC) Service is a specialist service for disabled children aged from birth to 19, and their families/carers. ITACC provides an integrated approach in supporting therapeutic intervention and care co-ordination for disabled children and young people with complex multi- disciplinary needs. ITACC works across four localities in Kent. The localities are: • Dartford, Gravesham and Swanley • Sevenoaks, Tonbridge and Tunbridge Wells • Maidstone and Malling • Thanet and Canterbury. In each locality, the ITACC teams are divided into early years and school age teams. The service to children in mainstream and special schools varies across localities, dependent on commissioning arrangements. ITACC teams consist of physiotherapists, occupational therapists, speech and language therapists, specialist nurse advisors, key workers, therapy assistants and administrators. Therapy provision in ITACC is delivered through co-ordinated packages of care to children with severe and complex needs, who are referred from a number of sources. The care co-ordination aspect of the ITACC team is provided by specialist nurse advisors and key workers. This is a designated service for children with severe and complex needs, aged from birth to seven.
