Length of Programs: Most weekly programs run for eight weeks, however, programs with this symbol meet prior to and/or after the regular program dates.

Min/Max Information: Program Min/Max numbers reflect the minimum number of participants required to run a program and the maximum number of participants allowed to register for a program. Some programs may be offered in more than one section of the program guide according to the designated age group. AIF/SIF Forms: Participants are required to complete an Annual Information Form (AIF), found on our website at www.nssra.org/forms. The AIF must be completed and filed with the NSSRA office with the completed registration form in order for a participant to join any NSSRA program or event. The AIF contains important participant information which is necessary for NSSRA staff to plan and execute safe and enjoyable programs. Please also complete the Seizure Information Form (SIF), if applicable. The SIF can also be found online at www.nssra.org/forms.

Payment must be included. You may enclose a check payable to NSSRA or charge the total amount on American Express, Discover, MasterCard or Visa. Please include card type, account number, expiration date, V–Code (on back of credit card) and your signature. Any outstanding balance must be paid before registration for next season can be accepted.

Register early. NSSRA program registrations are processed on a first come, first served basis. We understand that it is not always possible to meet a deadline. However, if we receive your registration after our deadline, your registration will be held until we can determine if accommodations for the participant can be made. These accommodations include transportation capacity, staff assignments and space limits at particular events. Please be assured that we will do our best to fit you in or find another suitable program.

InclusionNSSRA recognizes that some individuals with disabilities would benefit from social opportunities available by participating in programs offered through their local park district or recreation department. Inclusion provides a choice for individuals of varying abilities to experience recreation programming within their community. NSSRA and partner agency staff work closely with the participant and family to provide enriching recreation experiences in a least restrictive environment while maintaining safety and confidentiality.

Inclusion services are offered to residents of NSSRA’s 13 partner agencies at no additional cost to the family. Inclusion support may come in the form of one or more of the following: program observation, staff training, program modifications/adaptations, modified equipment, and/or an inclusion companion. An inclusion companion’s role is to blend into the program with the partner agency staff while providing the appropriate amount of support to promote independence and growth in a least restrictive environment. A companion can support 1-3 participants (depending on their needs). REQUESTING INCLUSION SERVICES When registering for a program with one of NSSRA’s partner agencies: • Mark the accommodation section of the registration form. This can be indicated in the following ways: Special Accommodations, ADA Information, or Americans with Disabilities Act.

• Once the registration form is submitted, the partner agency will contact the family to discuss the type of inclusion support needed I nc l usi o n for the participant. • The partner agency will provide NSSRA with an inclusion request that contains both program and participant information. • The family is then contacted by NSSRA to further discuss the needs of the participant and get input on the type of support needed. • NSSRA will work with the partner agency to put the necessary inclusion accommodations in place for participation in the desired program(s). • Communication between NSSRA, partner agency staff, and the family continues throughout the inclusion process. REASONS TO REQUEST INCLUSION SUPPORT • A parent or family member recognizes that a participant would benefit from additional support in a partner agency program. • The partner agency staff identifies a need for additional support, for safety and programmatic reasons and communicates this need to the family. Enriched Lifestyles for Adults (ELA) is a year-round day program for individuals with developmental disabilities who are 22 years and older. As a collaborative effort between The Center for Enriched Living (The Center) and Northern Suburban Special Recreation Association (NSSRA), ELA presents the best resources these two organizations have to offer. NSSRA and The Center each bring more than 40 years of experience in serving people with disabilities to this exciting collaboration. ELA was created to enhance the quality of life and offer well-rounded daytime program options for adults with disabilities while providing a flexible schedule for families.

ELA’s core program areas are: • Independent Living & Continuing Education: Computers, Nutrition & Health, Money Skills, Reading, Cooking • Communication & Socialization: Peer Interactions, Current Events, Relationship Building, Conflict Resolution • Recreation and Leisure: Sports, Fitness, Arts & Crafts, Music, Hobbies • Community Integration: Community Service Projects, Lunch Outings, Day Trips, Using Community Resources

ELA is held at The Center for Enriched Living, Monday-Friday, from 9:00 am to 3:00 pm. Participants may choose to attend two, three, or five days per week, with half and full day options available. Gator Athletics is the competitive division of NSSRA’s Athletics & Fitness programs. Participants competing on Gator teams are active in Illinois Therapeutic Recreation Section competitions and Special Olympics Illinois games. Gator Athletics gives NSSRA’s athletes the experience of competing against other individuals of similar abilities from nearby communities. Gator Athletics run on a different schedule than NSSRA traditional programs to meet deadlines established by Special Olympics Illinois.

It is important for athletes to attend and participate in all practices and games as well as tournaments. Athletes attending any tournament with NSSRA are required to register for this additional program with no extra cost. Registration materials will be sent home during the program and must be turned in by the deadline printed on the registration form. This registration is a commitment that the athlete will participate in the tournament.

Athletes who compete in Special Olympics Illinois competitions are required to have a valid Application for Participation on file with Special Olympics Illinois.

2016 General Camp Information PARENT INFORMATION Parent letters will be mailed approximately three weeks to one month before the start of camp. The letter will include site location and calendar of events. FINANCIAL ASSISTANCE AND PAYMENT PLANS Financial assistance is open to NSSRA residents who complete an NSSRA Reduced Fee Request Form and submit a $75 deposit with their registration. To receive a Reduced Fee Request Form or to request a payment plan, please call NSSRA Registrar Caitlin Deptula at (847) 509-9400 x6815. COMMUNICATION AND SAFETY Families are required to have a current Annual Information Form (AIF) and Seizure Information Form (if applicable) on file with NSSRA prior to participation in any camp or program. Please contact NSSRA to discuss specific accommodations for your camper. Communication Books will be used daily for each camper to help facilitate communication between families and staff. Communication books will be available at the Open House or sent home on the first day. WHAT TO WEAR AND BRING TO CAMP Please send campers in comfortable clothes and gym shoes each day. On swim days, campers should bring a labeled swimsuit and towel. Campers should also bring a labeled backpack, change of clothes and sunscreen each day. LUNCH, SNACK AND DRINK Parents are responsible for sending a labeled lunch and/or snack and drink each day. All items should be completely disposable. Campers should also bring a labeled water bottle on the first day to leave at the camp site. ANNUAL INFORMATION FORM (AIF) To better serve you and your child’s needs, it is necessary for us to have a current AIF on file. This form contains medical and other information that NSSRA needs to know in case of an emergency. If we do not have your AIF on file, your child will not be allowed to participate. If qualified, registration materials can be obtained from NSSED and/or your child’s school. This five week summer program includes education in the morning and optional recreation in the afternoon. Afternoon activities include sensory activities, fine and gross motor skill development, crafts, cooking, sports, indoor and outdoor games, field trips, socialization among peers and swimming! NSSED and NSSRA will place students at camp sites based on appropriate educational groupings and facility availability.

Staff include the NSSRA Recreation Directors, NSSED Education Directors, Special Education Teachers, Site Coordinators, Speech and Language Therapists, Teacher Assistants, Program Assistants and Volunteers. Nursing services are also provided. The staff to student ratio is 1:5 unless indicated as 1:1 in their IEP. Recreation staffing is based on camper needs and activities.

Goals: NSSED strives to maintain educational progress made during the school year. NSSRA’s goal is to provide campers a well-rounded summer experience through appropriate social interaction with peers, stressing individual independence and implementing an overall sense of well-being through recreational activities.

CAMP DUNCAN Registration Deadline: Friday, June 10 Start packing your bags because NSSRA is heading back to Camp Duncan for another year of fun! Camp Duncan is located in Ingleside, near Fox Lake, on 278 acres of land and is situated on Fish Lake. It is a fully accredited site through the American Camping Association. Camp Duncan offers many activities including boating, archery, hiking, team’s courses and an indoor Olympic size swimming pool.

Fee includes transportation, meals, accommodations and a wide variety of activities. New participants must attend an NSSRA overnight trip prior to the registration deadline for Camp Duncan so NSSRA can assess the individual’s readiness for this trip. Each week, the group will meet at Takiff Center where we’ll have an afternoon snack and play games while we wait for our friends to arrive. Once everyone’s ready, we’ll stay in for sports or head out for an afternoon adventure, bowling or swimming. SATURDAY SOCIALIZERS* Let’s start the afternoon off right! We will have lunch with our friends, discuss current events, and listen to some great music. Equestrian Connection is a one-of-a-kind stable that provides therapeutic horseback riding specifically designed for individuals with special needs. Participants will work to gain strength, flexibility and balance through riding. Each participant will learn how to ride, groom, saddle and tack a horse. During the program, the group will also be engaged in other horse-related activities and a snack will be provided each week. We will have lunch with our friends, discuss current events, and listen to some great music. Each week, we will find a new dessert, appetizer, or bread to make. The last part of our afternoon together will be spent hanging out and engaging in different leisure activities. Activities may include sports, playing board games, or making a craft. If the weather is appropriate, we may go outside so please dress according to the weather! Participants will also decide on various community outings that the group will attend throughout the season. If you register for a transportation option, please list your choice of pick up AND drop off locations on the registration form where indicated, using codes provided on page 59. If you would like to use NSSRA as a transportation location and it is not listed as an option, please contact NSSRA Recreation Specialist Mary Kate Murphy at (847) 509-9400 x6833 or [email protected].

If a participant is registered for transportation and will not need transportation pick up and/or drop off on a particular day(s), please notify NSSRA by calling the front office at (847) 509-9400. Routes are otherwise unnecessarily delayed.

DOOR-TO-DOOR ROUTES Door–to–door transportation is for residents only. Door–to–door routes are planned according to the number of participants registered and their locations, as well as the final destination. With limited vehicles available, it may be necessary for participants to ride in the bus for up to 1.5 hours. NSSRA staff will assist participants to and from their homes, but driveways and walkways must be clear of debris and obstacles. Staff will not enter homes. Persons unable to be in a house alone should have someone meet the bus at the designated time of return. NSSRA cannot wait more than ten minutes due to the demanding transportation schedule. Participants will be picked up and dropped off in geographical order. REC N’ ROLL TRANSPORTATION Transportation to and from NSSRA’s Rec N’ Roll after-school program must be coordinated by the family or guardian. Parents and/or guardians may drop off participants at the program location or work with the school to coordinate transportation services from school to the program location. Participants may arrive to the program location as early as 3:00 pm, but no later than 3:45 pm in order for the group to leave for the outing scheduled. Any participant who does not arrive to the program location by 3:45 pm will be required to meet at the outing location at a designated time or will be sent home. If the participant will be using transportation from school to NSSRA’s Rec N’ Roll program, the parent/ guardian must notify the participant’s school of the day and time the participant has registered for Rec N’ Roll. NSSRA will follow up with the participant’s school as listed on the Annual Information Form (AIF) to confirm the registration and location of the after-school program.

LATE PICK UP POLICY When parents/guardians/group home staff are late picking up a participant, drivers will wait a maximum of ten minutes from the scheduled time. After the ten-minute wait period, the driver will continue on the route to the next location. Participants/families who do not arrive on time may be asked to meet at an alternate location. After ten minutes, a fee may be applied.

PICK UP & DROP OFF SAFETY For the safety of all participants, parents/guardians/group home staff must personally check in with a program leader/driver when dropping off and picking up a participant. Staff cannot leave a program or other participants to escort an individual to or from a vehicle. Therefore, parents/guardians/group home staff are asked to park and check in with the program leader during both drop off and pick up for all programs.

PARTICIPANT TRANSPORTATION SAFETY NSSRA has implemented the following transportation procedures in order to ensure participants’ safety while riding in NSSRA vehicles. 1. All passengers must be sitting in seats or wheelchairs with seat belts securely fastened. The driver will not proceed until all seat belts are fastened. 2. All wheelchairs must have brakes that are in good working order and can stop the chair from moving. 3. The NSSRA drivers will assist participants to and from their homes, but driveways and walkways must be clear of debris and obstacles. 4. NSSRA drivers are responsible to determine whether or not a participant can be transported safely. If it is determined that it would be dangerous to transport an individual, the driver may refuse to transport that person.* 5. Persons riding in Amigo-type wheelchairs must transfer into a bus seat and must use a seatbelt. 6. Persons unable to be in the house alone should have someone meet the NSSRA staff at the designated time of return. NSSRA cannot wait more than ten minutes due to the demanding transportation schedule.
