Consultant in Old Age Psychiatry North West Area Centre, 40 hours per week £80,653 (GBP) to £107,170 (GBP) per annum Tenure: Permanent

Applications are invited for the post of Consultant in Old Age Psychiatry.

Following the retirement of the incumbent, we are looking to appoint a new consultant in Old Age Psychiatry in the East Ayrshire Locality. Serving a population of 10,500 Older people in a mixed urban rural catchment area, the successful applicant will join six other consultant colleagues in delivering a comprehensive mental health service for older people in NHS Ayrshire and Arran.

The post will be based at the North West Kilmarnock Area Centre with inpatient beds being situated in the new purpose-built facility at unit at Ayrshire Central Hospital in Irvine. The successful applicant will have consultant responsibility for a shared proportion of these inpatient beds. The community team is based at Rothesay House, .

As part of the “New Models of Care for Older People and People with Complex Needs” programme which is being developed by the three Health and Social Care Partnerships and NHS Ayrshire and Arran Acute Hospitals Division, elderly mental health services will be moving towards a truly integrated care model. The aim is to become active partners with not only patients, families and carers but by developing integrated working with health & social care partners.

Participation in the out of hours rota (1:23) is essential.

Consultant Psychiatrist Old Age Psychiatry

Background This is a well established post within NHS Ayrshire and Arran and this vacancy has come about due to the retirement of the incumbent.

This is a 10 PA traditional sector based consultant post based at North West Kilmarnock Area Centre in Kilmarnock covering the south half of the East Ayrshire Health and Social Care Partnership. Inpatient beds are now being accommodated in the new Woodland View inpatient unit at Ayrshire Central Hospital in Irvine. The community team base is Rothesay House in Cumnock.

The successful candidate will be encouraged to develop research interests and to further foster links with local universities.

Less than full time working would be negotiable.

The Psychiatry Service Facilities

In-patient mental health services in Ayrshire are provided from one main site, the new purpose built Woodland View inpatient at Ayrshire Central Hospital in Irvine.

Elderly Mental health beds are currently split between in and Woodland View. The direction of travel is that all acute inpatient beds will be consolidated on the Ayrshire Central campus in the short to medium term.

Hospital Based Complex Clinical Care beds for Elderly Mental Health are currently split across three sites; Ailsa Hospital in Ayr, East Ayrshire Community Hospital in Cumnock and Cumbrae Lodge Care Home in Irvine, immediately adjacent to Ayrshire Central Hospital.

University Hospital is a large, district general hospital in Kilmarnock. It has 600 acute hospital beds serving North and North-East Ayrshire.

University Hospital Ayr is a smaller DGH serving the population of south and south- east Ayrshire.

In addition there are Care of the Elderly beds based in Ayrshire Central, Biggart, Community and East Ayrshire Community Hospitals.

Future plans of the specialty It is envisaged that within the next five years, all Elderly Mental Health inpatient services will be consolidated on the Woodland View site. The successful candidate is expected to support this process.

Then “New Models of Care” work stream is due to re-start in 2018, with a move to more integrated working with primary care, Care of the Elderly services Health and Social Care personnel and, voluntary and third sector partners. The successful candidate is expected to be intimately involved in planning and delivering new ways of meeting the care needs of the elderly population within this sector.

The post Duties and Responsibilities The main duties and responsibilities of the post include:

Clinical Provision  Medical responsibility for the provision of Elderly Mental Health Services within NHS Ayrshire and Arran into the south locality of the East Ayrshire Health and Social Care Partnership  Lead diagnosis, treatment and management of older patients with mental health difficulties within this locality.  Provide consultant responsibility for inpatients admitted to the Woodland View inpatient unit from this locality  Provide twice weekly outpatient clinics for patients from this locality  Provide Consultant leadership to the multidisciplinary team through support, advice and guidance on clinical mental health issues to assure high quality patient interventions.  To provide consultant input and supervision to the care home liaison nurse in order to, where possible, avoid inappropriate admission to both general hospital and mental health beds.  Develop effective working relationships with partner organisations  Maintain compliance with legal requirements covering all aspects of clinical management to support NHS Ayrshire and Arran to meet its statutory requirements in delivering safe, efficient patient services.  The successful applicant will be encouraged to participate in research and to develop a relevant subspecialty interest, subject to resources and in line with local priorities.

Education and Training Key individual learning outcomes will include taking part in Continuing Professional Development within the framework of the Royal College of Psychiatrists to support maintenance and development of clinical competence and performance.

The post holder will require to engage in training for Clinical & Educational supervision.

Clinical Governance  Contribute to and participate in driving the clinical governance agenda within Elderly Mental Health, emphasising high quality clinical interventions in all aspects of patient care.  To represent the specialty as required and participate in the development, implementation and review of NHS Ayrshire and Arran Clinical Governance frameworks and related initiatives as required by Mental Health services.  Participate in Clinical Audit & other quality improvement work and review outcomes, ensuring that the service progresses towards achievement of national and local performance management targets, complying with risk management policies and related guidelines.  Provide and circulate reports of assessments and the management of patients to professionals and partnership stakeholders who require the information in accordance with confidentiality and data protection guidelines.  Maintain patient notes to the required standard and monitor quality of clinical notes both electronic and written in accordance with data protection and Caldicott principles.  To contribute to National Policies pertaining to Elderly Mental Health Services.

Administrative Duties  To undertake the administrative duties associated with the care of patients ensuring principles of good medical practice are observed at all times.  To ensure that clinical activity is accurately and comprehensively recorded and submitted to the Information Department to facilitate collation, reporting and review of clinical performance.

Management Duties  Support colleagues and stakeholders to achieve the performance targets and commitments specified within “Scotland’s Mental Health Strategy 2017-2027” and “Scotland’s National Dementia Strategy 2017 -2020” and NHS Ayrshire and Arran’s local delivery plan, to facilitate service transformation.  Contribute to Service management by providing support to the ongoing development of the service through appropriate special interests and supporting strategic service priorities.  Maintain attendance at appropriate senior medical committees and forums and actively participate in the agenda and development initiatives to align the service with strategic and clinical priorities.  Manage doctors in training in the performance of their duties and to support ongoing professional education, development and welfare support to ensure

compliance with the principles contained within NHS Education for Scotland guidance.  To observe the Division’s agreed policies and procedures, drawn up in consultation with the profession on clinical matters, and to follow the standing orders and financial instructions of NHS Ayrshire & Arran  To work as a member of the Elderly Mental Health Medical Management Team and attend and contribute to a monthly meeting of this Group.

The post is within the Mental Health Directorate. The post holder will be accountable to the Clinical Director for Elderly Mental Health Services, who will agree the Job Plan.

For professional management of education activities, the post holder will be accountable to the Assistant Director of Medical Education.

Subject to the provision of the Terms and Conditions of Service, the post holder is expected to observe NHS Ayrshire and Arran’s agreed policies and procedures, drawn up in consultation with the profession on clinical matters, and to follow the standing orders and financial instructions of NHS Ayrshire and Arran.

Notes on the Programme Patient Administration This activity covers the management of individual patients including Out Patient administration, results reporting, letters/phone calls to patients, carers, GP’S and members of the wider multidisciplinary team involved in the patients care.

Ward Rounds The time allocated is indicative and will be discussed with the appointee. Ward work will include teaching ward rounds as required.

Travel Any travel allocation will be included within the Total Programmed Activities and will be determined by location at which Direct Clinical Care and Supporting Professional activities are carried out.

Supporting Professional Activities NHS Ayrshire and Arran recognise the important role Job Planning has in ensuring consultants are supported in delivering high quality, safe, sustainable clinical care to patients. All consultants will have 1 SPA as a minimum to support job planning, appraisal and revalidation. However the final balance of SPA and DCC activity will be agreed between the appointee and clinical manager prior to contracts being agreed.

There may be a requirement to vary the DCC outlined in the indicative timetable below when the final balance of DCC and SPA is subsequently agreed. There may also be opportunities to contract for Extra Programmed activities Opportunities subject to service requirements and in accordance with national terms and conditions of service.

If the post-holder will be responsible for the formal training and supervision of post- graduates and under-graduates, a suitable additional allocation of SPA time will be made in accordance with national guidance.

Job Plan Review New appointees will discuss the indicative job plan with the Clinical Director, prior to commencement and will at that time review the balance of activities. Where it is possible to agree a revisions to the indicative plan in advance of commencement this will be acted upon. In any event however, there must be an interim Job Plan review conducted at 3 months post commencement to agree and finalise the Job Plan. The consultant at time of induction should ask for an interim review date to be scheduled. The agreed job plan will include all the consultant’s professional duties and commitments, including agreed Supporting Professional Activities. Thereafter Job Planning will be carried out annually as part of the Boards Job Planning process.

Private Practice If the post-holder wishes to undertake any private practice, they are obliged to inform their employer at the time of appointment of their intentions to do so. This should be submitted in writing to the Clinical Director. The conduct of private practice will be in accordance with the Consultant Contract (Scotland) Terms and Conditions.

The post-holder shall be free to undertake private practice without approval provided such work is undertaken outside the time agreed in the job plan for programmed activities.
