Line Management To provide clinical leadership to the keech hospice care medical team Responsibilities:

Hours: 8 to 10 PAs (4 to 5 days)

Responsible for providing medical leadership, support and guidance across Keech Hospice Care, and Luton community services. The post Main purpose of holder will work with the palliative and end of life multidisciplinary team to role: ensure patients receive responsive and effective management of their palliative care needs, and that care is seamless across organisations. To work collaboratively with palliative care consultants and medical teams across the BLMK STP to support best practice across all settings Keech Hospice Care and Cambridge Community Services (CCS) work in an integrated way within Luton to provide palliative and end of life services to patients, their carer’s and families. The organisations have come together to employ a palliative care consultant who will enhance the pathways. Keech Hospice Care is an independent hospice providing specialist palliative and end of life care for adults across Luton and South and children across Bedfordshire, and Milton Background: Keynes. The adult services include an 8 bedded inpatient unit, an outpatient facility, My Care Coordination Team, rare neurological CNS, supportive and social work team, aquatic therapy, the Independence and Wellbeing Service and a team of medical speciality doctors. (This role is to support adult services). Cambridge Community Services NHS trust provides a team of palliative care specialist nurses, lead palliative care nurse and the wider generalist nursing and AHP teams for the Luton community.

1. DIRECT CLINICAL CARE (DCC) 1.1. To provide medical leadership and accountability for the medical care of adult patients accessing hospice services, and in the Main duties/ community. Working with the multidisciplinary team, reviewing responsibilities: individual patients in their place of care and developing individualised plans of care 1.2. Providing home assessments either jointly with the specialist palliative care nurse, or GP, or independently, agreeing a plan of

care and communicating this with primary health care team via electronic patient records. 1.3. Manage and supervise the medical team and support the specialist nurses and multidisciplinary team 1.4. Provide timely and direct liaison with Hospital Consultants, GPs, Community Nurses, and non-medical Prescribers after assessing unstable and/or complex patients on IPU, and in the community 1.5. To promote a seamless and coordinated system of palliative support across all Hospice departments and community settings 1.6. Engage and support patient/carers and families with managing complex clinical, emotional and social situations. Compassionate, caring and able to manage essential conversations 1.7. Have a high level of cultural sensitivity and cultural competency 1.8. Provide input into the Hospice multi-professional case discussions and the community teams (CCS) case discussions at internal MDTs and operational meetings 1.9. Lead on development of Clinical protocols and liaising with the hospital palliative care team develop pathways that reduce unnecessary admissions and reduce hospital stay 1.10. Participate in weekly multi-professional team meeting and ensure in collaboration with the hospital palliative care consultant, that consultant representation at this meeting is always available 2. SUPPORTING PROFESSIONAL ACTIVITIES 2.1. To regularly participate in audit, data evaluation, and research projects relevant to the service delivery at the Hospice and community palliative care service. Support and develop work undertaken by professional colleagues 2.2. To participate in the clinical governance processes of the Hospice to ensure the safe and effective delivery of care 2.3. Participate in the education and training of, healthcare professionals and students, as well as volunteers (as appropriate) 2.4. Comply with CPD requirements as stipulated by the Royal College of Physicians (RCP) and in line with the Appraisal and Revalidation requirements 2.5. Undertake management, appraisal and where appropriate, assessment of colleagues and junior staff 2.6. Participate in personal annual appraisal, review of job plan, and Medical Revalidation 2.7. Organise and chair the monthly medical meeting held at the Hospice to discuss clinical, operational and strategic issues

2.8. Represent the hospice and locality at regional and national palliative and end of life care meetings 2.9. Work collaboratively with local providers of specialist palliative care, community NHS and social/voluntary services, attending the palliative and end of life local implementation group (LIG) 2.10. To work collaboratively with palliative care consultants and medical teams across the Bedfordshire, Luton, Milton Keynes STP (BLMK STP) to support best practice across all settings, leading on clinical developments and innovative cross organisational working 2 EDUCATION 3.1. Teaching requirements include: • Regular teaching sessions for the multi-disciplinary team as part of a programme of professional development • Teaching ward rounds on in patient unit (IPU) • Working with GPs and primary care networks to develop relevant education for primary care teams • Regular teaching across the locality on all aspects of palliative and end of life care for both malignant and non-malignant diagnoses (specialist and generalist) • Face to face teaching of GP Registrars, placement doctors and other health care professionals • Mentoring of Non-Medical Prescribers during their training

The postholder will review, with the medical team the 8 in patients within the hospice, provide clinical leadership to specialist palliative care nurse colleagues and doctors, and directly support the management of up to 12 patients within a community setting. This may vary, depending on patient numbers, but often support will be provided to professional colleagues in the management of palliative patients.

The post holder will be provided with administrative support and provided with a laptop to ensure agile working is possible. The base is keech hospice care, where office accommodation and admin support will be provided (wifi and IT support available), in addition the post holder will spend time hot desking (wifi available) in community services (CCS) operational base in Luton. (Poynters RD, Luton) The post holder will need to drive between locations and conduct home visits. Ongoing support to the post holder will be provided by the Clinical Director, keech hospice care, with mentoring and guidance provided by the Medical Director from Cambridge community services Currently there is no on call requirements, but this may change to support service development. If on call becomes a requirement, time off in leu of this will be factored into the work plan.
