Deprivation Indices

The deprivation indices as set out in the paper: (McIntyre and Okorafor 2003) were recalculated to sub-district and district level for use in this District Health Barometer. The deprivation index is a measure of relative deprivation across districts within South Africa. Just as any index, the deprivation index is a composite measure derived from a set of variables. The deprivation index used in this report was generated using principal components analysis (PCA). PCA identifies the underlying process that has the most influence in determining the outcome of each variable included in the analysis. Each variable is weighted based on its linear association with the underlying process. The weighted variables are then used to construct the deprivation index. The deprivation index can therefore be viewed as a standardised measure of this underlying process that influences the outcomes of each variable (included) in each district. Variables included in the analysis are variables considered to be indicators of material and social deprivation. The major underlying process that influences the outcomes of these variables is social and material deprivation – hence a deprivation index. Details regarding the interpretation of the deprivation index can be found in the sections below.

Technical information on the deprivation indices used in this report

Variables included for calculating the deprivation index were: 1. The proportion of the area’s population that are children below the age of 5 2. The proportion of the area’s population that are black Africans 3. The proportion of the area’s population that are from a household that is headed by a female 4. The proportion of the area’s population whose household heads have no schooling 5. The proportion of area’s adults between 25 and 59 classified as both not working and looking for work or not working and not looking 6. The proportion of the area’s population that live in traditional dwelling, informal shack or tent 7. The proportion of the area’s population that have no piped water in their house or on site 8. The proportion of the area’s population that have a pit or bucket toilet or no form of toilet 9. The proportion of the area’s population that do not have access to electricity or solar power for lighting, heating or cooking

Geographic considerations: ◆ District Management Areas (DMAs) such as national parks, game reserves and wilderness areas were not included in the calculation of the indices. ◆ For all health districts, a single index is generated, even though they may be cross-border districts and fall into two provinces. For example, Greater Sekhukhune DM appears to be evenly split between Limpopo and Mpumalanga. A single deprivation index is generated for Greater Sekhukune DM, and is listed under Limpopo Province. ◆ Bohlabela DM and its local municipalities (Bushbuckridge LM and Maruleng LM) are listed under Limpopo. ◆ Frances Baard DM is listed under Northern Cape, and so are all the local municipalities that are within it. ◆ Merafong City LM is listed under West Rand DM, under Gauteng. ◆ Kgalagadi DM and the local municipalities that are under it are all listed under Northern Cape. ◆ Moshaweng LM is coded under North West in the census data. Although it falls under Kgalagadi DM, it is listed under North West.

Normalising the deprivation index

The deprivation index scores produced usually ranges from negative values to positive values. Higher values represent higher levels of deprivation, and lower values represent lower levels of deprivation. For example if the deprivation index scores range from -3.564 to +3.687, then the district with -3.564 is the least deprived district. By the same token, the district with +3.687 is the most deprived. Interpreting negative and positive values can be quite involving, so the deprivation index was normalised. This means that the indices were moved to the positive number scale. Consequently, the district with a deprivation index score of 1 is the least deprived. Districts with higher values are relatively more deprived than districts with lower values (the same applies to sub-districts). For example, Umzimkhulu LM in Alfred Nzo DM has a deprivation index value of 4.028, while Umzimvubu LM has a value of 3.974. This means that Umzimkhulu is more deprived than Umzinvubu. The values of the deprivation index for any area is generated relative to all other areas (whether category A, B or C municipalities). Therefore one can rank local municipalities and district municipalities according to their deprivation index scores.

Quintiles for the normalised deprivation indices

Separate spreadsheets were generated for health districts (category B municipalities including metropolitan areas) and category C municipalities (sub-districts). For each of the data sets, the municipalities were ranked according levels of deprivation, and were categorised into quintiles. With reference to the spreadsheet for category B municipalities (health districts), the districts that fall under quintile 1 (lowest quintile) are the most deprived. Those that fall under quintile 5 are the least deprived (best-off). Each quintile contains 20% of all districts. The same applies to the spreadsheet for sub-districts (category C municipalities).

The District Health Information System

The District Health System (DHIS) is a key element in the overall national health information system in the public health sector. The DHIS uses software which allows clinics and hospitals to enter data relating to their services. The data is then transmitted electronically to province and national departments.

The diagram above illustrates the flow of data from facility to national level. The system uses a transient database from which users can generate reports. Reports can be tailored to include certain indicators. Health indicator sets can also be interfaced with the free Arc Explorer software that allows data to be presented as thematic maps or analysed further in Geographical Information System software. The generation of pivot tables is another tool that allows data to be presented in various ways. The DHIS software supports not only routine monthly data, but also the capture and analysis of semi- permanent data (e.g. population estimates, equipment, infrastructure, number of personnel) and survey data. This stems from the premise that not all information needs to be collected on a routine basis. Tuberculosis Surveillance and data collection in South Africa

Background

The South African government launched the revised National TB Control Programme (NTCP) in 1996. In establishing the NTCP, the government signaled its adoption of the WHO DOTS Strategy (Directly Observed Therapy, Short-course) to fight TB and set about the task of standardising procedures related to TB case diagnosis, treatment, and reporting. An essential element of the NTCP is an efficient Recording and Reporting System. This serves as a tool for programme managers at district, province and national levels to evaluate progress made in achieving targets, supervision and deployment of staff and for monitoring and surveillance.

The Tuberculosis Reporting and Recording System

The original recording and reporting system was developed by the MRC Tuberculosis Research Programmes in 1994, in close collaboration with the Directorate Health Systems Research, Research Co- ordination and Epidemiology and the Directorate: Communicable Disease Control of the Department of Health, and implemented during 1995 and 1996. This system was called the TBSYS system and was based on the manual registration of patients in facilities and then quarterly aggregation of reports at facility level, summarised at district level and entered into an electronic database at provincial level. The shortcomings in the system included that it was an aggregated facility-based system and did not allow for cohort analysis. There was also no link between case findings and treatment outcomes. In addition, records of ‘Patients Transferred Out’ were excluded from the denominator in the outcome report. The implementation of the new national strategy on district health systems provided the opportunity to revise the recording and reporting system. The new TB strategy coincided with the Department of Health’s adoption of the district health system and a commitment was made by the Director-General to implement a district-based TB reporting system to replace the previous facility-based system. In addition, the system was adapted to conform to the standard WHO/IUATLD recording and reporting system and to incorporate the standard international definitions. During August 1999, the BOTUSA project gave a demonstration on an electronic TB Register (called the BOTUSA system) that had been successfully developed and implemented in Botswana. This lead to the BOTUSA system software being piloted in Mpumalanga and North West provinces during 2000. This electronic TB Register forms part of the paper-based tools in TB. Key elements of the ‘new’ Recording and Reporting System, include: a. Collection of individual, patient-based data b. District-based cohort analysis on a quarterly basis, using standardised formats and definitions c. Interim reports (smear conversion) and treatment outcome reports based on quarter of diagnosis d. Calculation and analysis of key performance indicators e. Use of data at facility and district-levels for performance assessment and improvement. With the introduction of the electronic TB register, the number of patients that do not have a registered outcome, can be identified. These patients show up as ‘Not Evaluated’ in the outcome report. As not all provinces changed over to the new system simultaneously, outcome reports up to the end of 2003 excluded the ‘Not Evaluated’ patient count from the denominator. This changed in 2004 once all nine provinces were on the same system and has caused some provinces’ data to show a ‘deterioration’ in cure rates due to the inclusion of a high proportion of patients without recorded outcomes in the denominator.
