TQ Padlocks & Keys

The Permit Holder must, and must ensure its subcontractors and agents, take all reasonable care and diligence at all times to prevent any loss of any Transurban issued property, including any keys or padlocks provided by Transurban to the Permit Holder for the purposes of Accessing secure areas of the Asset. The Permit Holder and any of its subcontractors or agents must keep Access Gates Locked at all times, and secure keys on their physical person at all times. The Permit Holder acknowledges there is significant cost and inconvenience to Transurban if gates and padlocks are not secure and Transurban will be required to replace any lost locks and key (including multiple locks and keys across multiple locations the key is used) on an urgent basis to ensure these locations remain secure. The Permit Holder agrees that any theft or loss of Transurban property, including keys or padlocks by the Permit Holder, its subcontractors or agents is at the Permit Holder’s risk and the Permit Holder will be liable for any and all costs of Transurban associated with and incidental to such replacement.

Communication

All work that has the potential to have an impact (minor, moderate or major) on motorists and/or the community (i.e. residents in close proximity to works) as defined in Transurban Queensland Stakeholder Engagement and Community Relations Framework requires communication. Transurban Queensland Community Relations ( [email protected] ) must be advised 20 business days in advance of these works in accordance with the Framework so the appropriate level of communications can be implemented.

Project Delivery

The Applicant (not associated with a Transurban Queensland Major Project) is required to interface with any Principal Contractors appointed by Transurban Queensland working within or around the proposed works area. Details of Principal Contractors can be provided to the Applicant by Transurban Queensland.

Corridor

The Applicant must notify the Transurban Queensland Traffic Management Centre immediately of any identified or suspected unauthorized access. The TMC should also be advised of any unauthorized waste material including items such as syringes found on site. No commercial advertising is permitted at any time including on boundary or temporary fencing.

Logan Enhancement Project

The Applicant must contact the Principal Contractor for the Logan Enhancement Project, CPB Contractors (Jens Beicht – [email protected]), if planning to conduct works on the following:

1. Logan Motorway ( from 2.5km east of Wembley Road to Centenary Motorway); or

2. Gateway Extension Motorway (between Pacific Motorway and the Logan Motorway).

The applicant must reach agreement with the Principal Contractor regarding site access, induction arrangements and any other requirements of the Principal Contractor at least 14 days prior to organising works in the vicinity of the above locations. General Conditions of ATW Permit

1. These general conditions (General Conditions) to permitted works on any Transurban Group operated asset, facility or corridor (Asset) apply in addition to any existing contract, agreement or other conditions, including any Transurban Group stakeholder engagement obligations, notified to it by the Transurban Group entity that manages / operates the relevant Asset (Issuing Authority) as being applicable to the Proposed Activities or the ATW (Existing Conditions). In the event of any inconsistency, the Existing Conditions prevail to the extent of that inconsistency. 2. No work or activities shall commence on an Asset until an approved Authority to Work Permit (ATW) is issued by the Issuing Authority to the Applicant (Permit Holder) outlining the approved work or activities (Proposed Activity). No work or activity is to be performed on an Asset unless those works or activities are Proposed Activities and are performed by the Permit Holder. The only personnel permitted to access the Asset are those working under the ATW (Personnel). 3. The ATW is only valid within the dates specified in the ATW, and no extensions are allowed. 4. As a condition of accepting this ATW and being granted access to the Asset, the Permit Holder agrees to these General Conditions and must procure that its subcontractors and Personnel agree with and work in accordance with these General Conditions. All Permit Holders, subcontractors and Personnel must comply with the ATW, these General Conditions and any additional conditions notified to it by the Issuing Authority when accessing an Asset. The Issuing Authority may nominate any person or company as the Asset representative (Asset Representative) at its sole discretion. 5. By entry into the ATW process, the Permit Holder agrees to take all reasonable care and diligence to prevent any damage to property or equipment in which the Issuing Authority and/or a member of the Transurban Group (as applicable) have an interest, and agrees to indemnify the Issuing Authority and/or a member of the Transurban Group (as applicable) for the cost of any repairs or replacement arising from the Permit Holder’s (or their subcontractor’s or agent’s) failure to do so. 6. Any breach of the ATW or these General Conditions may result in any Personnel’s or Permit Holder’s or contractor’s work being stopped and/or the Personnel or Permit Holder or contractor being removed from the Asset and/ or the ATW being revoked or cancelled, at the discretion of the Issuing Authority or Asset Representative. Neither the Issuing Authority nor the Asset Representative, nor any member of the Transurban Group will be responsible for any costs arising from or in connection with work being stopped and/or any person’s removal from site and/or revocation or cancellation of an ATW as a result of a breach of the ATW or these General Conditions. 7. In the event of a removal from site or a revocation or cancellation of the ATW, the Permit Holder shall immediately, and ensure its subcontractors and Personnel shall immediately, cease all activities, restore traffic (if applicable) and return to the Traffic Control Room all keys and other property of the Issuing Authority. 8. In the event of a major traffic incident, extreme weather event, or other incident on the Asset the Traffic Control Room may delay, suspend or cancel works or access to site at short notice. 9. The Permit Holder must notify any relevant government agency or authority (as applicable) of the Proposed Activity and obtain, and ensure that its contractors and Personnel obtain, any other required permit and/or approval before any Proposed Activity commences. The Issuing Authority may request a copy of any such permit and/or approval. 10. The Permit Holder warrants that all information and details on the ATW are accurate and complete and that it will notify the Issuing Authority of any change to that information immediately, including any change in the operating conditions (operating times, dates, duration). Any amendments to the ATW are to be provided to the Issuing Authority or Asset Representative at least 2 working days prior to commencement of any work or activity and a re-submission of and approval of the ATW may be required. 11. The Permit Holder must carry a copy of the ATW and any supporting documentation at all times when work is in progress on an Asset and ensure that all Personnel carry a copy of their relevant licences, qualifications and photo identification. These documents must be presented to an Asset Representative upon request. 12. The Permit Holder is responsible for all aspects of site control and safety for the works specified in the ATW and/or performed by it, its subcontractors or Personnel on the Assets. 13. The Permit Holder is responsible for planning and coordinating the Proposed Activities and is responsible for all aspects of site control and safety for the Proposed Activity. The Permit Holder must consider any information about hazards, risks and site conditions notified to it by the Issuing Authority, implement appropriate controls for those hazards and risks and provide appropriate information and instruction to subcontractors and Personnel about those hazards, risks and controls and the overall works being performed. ------····································································································································

All Personnel and the Permit Holder must: a. conduct a safety and environmental review of all hazards and risks associated with the work and work site before commencing work; b. attend any site safety briefing before the Proposed Activity commences (including any formal site briefings prior to tunnel closures or specific safety briefings); c. obey all signage installed throughout the Asset site; d. comply with the Transurban Personal Protective Equipment Procedure PRHSE001, the Transurban Alcohol and Other Drugs Policy HSE-PO-0003 (including submitting to any drug or alcohol testing conducted under that policy) and any other policy, procedure or site requirement notified by the Issuing Authority; e. Return any radios, keys or other items loaned to them by the Traffic Control Room. 15. No hazardous substance or dangerous goods (including gases and chemicals) firearms, ammunition, explosives or animals are to be brought onto site without undergoing a risk assessment and obtaining prior written approval of the Issuing Authority. At no time shall pyrotechnics, illicit drugs or alcohol be brought on to the site. 16. The Permit Holder is not permitted to erect, install, paint or display any advertising, promotional or similar signage or material on, in or near any part of the Asset or permit any other party to do so. Construction fencing and screens are not to include commercial advertising including company logos (with the exception of company logos on site information signage). 17. Photographs or films (including the use of drones) are not to be taken on site without prior written permission of the Issuing Authority. 18. Certain tools are banned from site (such as 9” grinders). Personnel must seek advice from the Asset Representative to confirm intended handheld tools are allowed on site. 19. To the extent practicable, the work site is to be maintained in a tidy and neat condition during the works and returned to its original state once works are complete. For ongoing works, the work site must be left in a clean and safe condition all doors and gates are to be closed and locked before leaving the work site and all lights are to be turned off in building areas. All tools and materials to be removed at completion of works. 20. For works on road surfaces, kerbs and channels, those areas must be kept clean, safe and free from debris. Construction material must not be able to enter the drainage system via open pits. 21. All vehicles and mobile plant are to be fitted with an orange flashing beacon light. All mobile plant and large vehicles/trucks are required to be fitted with a reversing beeper. 22. If the Proposed Activities include traffic control: a. The Permit Holder must ensure that all traffic control work (including any traffic control work carried out by its subcontractors and Personnel) complies with all requirements made or imposed under any applicable laws, regulations, codes, guidelines, industry standards etc. or by any applicable authority; b. The Permit Holder must monitor the surrounding traffic flow and should significant queuing be observed (i.e. the queue extends to the first set of roadwork signs), the Permit Holder should contact the Traffic Control Room, advise of conditions and jointly make a decision on best course of action to limit impact to traffic; 23. The Permit Holder must comply with any direction of the Issuing Authority, Traffic Control Room, the Asset Representative and/or the Coordinating Party (being the party responsible for overall co-ordination of HSE interfaces where there are multiple packages of works being carried out in close proximity) (if applicable). 24. In addition to any other requirement under these General Conditions or otherwise notified to the Permit Holder by the Issuing Authority or Asset Representative, the Permit Holder must ensure notice is provided to the Traffic Control Room of the following: a. On the commencement of Proposed Activities, where possible, at least one hour before work starts. b. Upon completion of the Proposed Activity at the end of each shift/day or after all traffic control devices have been removed; c. Immediately in the following circumstances: incident/accident/near miss/injury (including first aid injury); environmental harm or potential harm incident; spills; traffic queuing or traffic incident; security incident (including theft or attempted entry); attendance on site by the media; attendance on site by an authority (e.g. government departments, police, WorkCover, environment department representatives etc); presence on site of an unknown party (e.g. pedestrian, other contractor); unauthorised incursion into a work area (e.g. closed lane) by members of the public (motorists); site damage or graffiti. 25. All emergency access areas, pathways to the Asset facilities, including emergency gateways, emergency cross passages and emergency stairs must be kept clear at all times. 26. In the event of an emergency, all personnel must follow the direction given by the Traffic Control Room, incident response officers and Emergency Services (i.e. Police, Fire Brigade and Ambulance)

Traffic Impact Conditions

1. Auxiliary Flashing lights must be turned on at least 200m before exiting the motorway, including on and off ramps. Auxiliary Flashing lights must remain on when entering the motorway, including on and off ramps, until the vehicle is travelling at the sign posted speed. 2. No crossing of a live lane of traffic is allowed at any time. If access to a traffic lane is required then a Traffic Guidance Scheme (TGS) compliant to the latest MUTCD is required. 3. Lane closure permitted from Sunday - Thursday between the hours of 20:00 and 05:00. Unless agree to by TQ 4. Additional Traffic Guidance Schemes (TGS) for work impacting on and/or off ramps and main roadway shall be submitted three working days prior to allow Transurban to assess their suitability. Written confirmation from Transurban of the acceptance of the TGSs is required prior to implementation. 5. Any amendments to the original application and Traffic Guidance Schemes are to be provided to the Network Traffic Specialist through [email protected] for approval at least two working days prior to commencement of any work/activity. 6. No installation of portable VMS devices without written approval, VMS to be installed outside the clear zone (min 9 m) or behind guardrail and meet the requirements of the latest MUTCD Part 3 as well as the TRUM manual. 7. A minimum of 3.5 metres wide travelled path must be provided for vehicles when conducting partial ramp closures for a 40km/h speed limit. If this distance cannot be achieved, the Applicant shall submit an request for a ramp closure and include a site specific traffic guidance scheme and detour map 5 business days prior to works commencing to [email protected] . Any detours must provide for heavy vehicles and have the appropriate permits from other Road Authorities. 8. Roadside access 1m minimum clearance behind guardrail/PCB or 6m where no barrier treatment is available if this distance cannot be achieved, the applicant shall submit a site specific traffic guidance scheme showing worker protection methods. When working at the base of fill slope these distance shall be increased by minimum of 6m or protective barrier system installed for workers safety. 9. The Applicant/Contractor must monitor the surrounding traffic flow and should there be any significant queuing (i.e. the queue extends to the first set of roadwork signs), the Applicant/ Contractor shall clear the area and allow traffic queues to clear before resuming work/activity. In these circumstances, the Applicant/Contractor shall phone the Transurban Queensland Traffic Control Room and advise when the site is cleared and when the works resume. 10. The Applicant must engage qualified traffic controller/s to assist in the traffic control. 11. All lane closures are subject to weather conditions. 12. A minimum of 3.5m travel path must be maintained at all times during lane closures 13. Maximum single length of carriageway with a 40kph speed shall be no more than 1km 14. Signs for long-term aftercare work should be mounted on normal fixed supports so that they are not likely to be disturbed by the weather, vandals or traffic In open road areas and on unkerbed roads in built-up areas where signs are to be mounted on posts, they should normally be placed clear of the outer edge of shoulder and at least 2 m clear of the travelled path, whichever is the greater clearance. They should be erected 1 to 1.5 m above the level of the nearest edge of the travelled path to the underside of the sign.
