Your child has been selected by their school to take part in the University of Portsmouth’s Step UP scheme. We would ask you to complete this form so that we have appropriate information about your child/ward. Data Protection:

The Recruitment and Outreach team require the following data of your child (*optional):

-name -date of birth -name of pupil’s school -email address -gender -home postcode -ethnicity information* -disability information*

The Recruitment and Outreach team will use the data:

-to send participants information on services and resources offered by the University of Portsmouth -to communicate details of all events and activities appropriate to the participant -to track destinations of participants into higher education up to the age of 30 -to report on the outcomes of activity delivered by the University of Portsmouth -to request participants’ engagement in future activities, research or resources -for research and monitoring purposes

The data will not be used in a way that would affect you or your child individually. We will not release data to anyone who is not authorised to receive it (see below), and only those members of staff within the University who are involved with the Step UP scheme will have access to the data you provide. This data may be shared with the Higher Education Funding Council for (HEFCE), Higher Education Statistics Agency (HESA), the University and Colleges Admissions Service (UCAS) and the Higher Education Access Tracker (HEAT) and its service subscribers to help evaluate the effectiveness of this activity as part of the government policy to widen participation in higher education and to develop future policy. To further monitor the effectiveness of the programme, attainment data for individuals participating in the programme will be requested from your child’s school. This will be data already collected by the pupil’s school showing the pupil’s progress in subject areas and attitudes to learning. By agreeing to your child’s participation in the scheme you are allowing this data to be shared with the University of Portsmouth.

The data will be stored until your child reaches their 30th birthday. At the end of this period data will be deleted by the University of Portsmouth in accordance with national guidance.

Participant data will be stored in accordance with the Data Protection Act 1998 and used for research (to improve the work that we do) and monitoring (to measure the impact of our work) purposes. The data will be stored on servers based in the UK and EU.

Your rights: We are asking for your consent to collect and process this personal and sensitive data. If you do not give consent, this does not affect your child's participation in the programme. If you do give your consent, you have the right to withdraw that consent to either the processing of this data or to your child being involved in the Step UP scheme.
