1.0 INTRODUCTION

Bridge No. 411, constructed in 1941, is a 10’-0” x 10’-0” x 45’-9” long RCB culvert with flared and warped wing walls, concrete lined floor (to the end of the wing walls) upstream and downstream. The two lane road Is AC paved over unspecified depth of fill. There is 6” x 6” timber rail mounted on 8” x 8” timber post. There are Type OM-3L/R markers at all four corners and Type OM2-1V markers with the bridge number at the approaches. O’Leary Creek is a medium to high flow well defined rock and boulder channel. There is pipe and wire on the south embankment and grouted rock on the north upstream embankment. There is pipe and wire on the north bank downstream. The embankments and bottom have heavy vegetation and trees.

2.0 EXECUTIVE SUMMARY

Based on the information gathered from the inspection and following NBIS standards, this structure is rated in overall Good Condition.

The bridge is rated using the National Bridge Inspection Standards (NBIS) rating system. The NBIS rating system requires the each element is rated as good, fair, or poor, and that each major feature receive an overall condition rating from zero to nine. A condition rating of zero signifies a failed condition. A condition rating of nine signifies an excellent condition. The condition of the deck, superstructure, and substructure was assessed using this system. The approach roadway alignment condition is based on the adequacy of the approach roadway alignment. A condition rating of 3 or less indicates the horizontal or vertical curvature requires a substantial reduction in the vehicle operating speed. A condition rating of 6 indicates a very minor speed reduction is required and a condition rating of 8 indicates no speed reduction is required. O’Leary Creek does not require scour critical inspection. The sufficiency rating formula evaluates highway bridge data by calculating four separate factors to obtain a numeric value which is indicative of the bridge sufficiency to remain open. Ratings range from 100 (entirely sufficient) to 0 (entirely insufficient or deficient). Bridge No. 411 Sufficiency Rating was calculated at 76.59. Load Rating Analysis is not required.

3.0 INSPECTION FINDINGS

3.1 Approach Roadway Item 72 Code: 8

Approach alignment is on a slight curve to the left advancing north with dirt and gravel shoulders. There are no concrete approach slabs. The AC wear surface is in good condition (Appendix A Photo 1).

3.2 Traffic Safety Features Item 36 Code: 0 0 0 0

There is 6” x 6” timber rail mounted on 8” x 8” timber post (Appendix A Photos 1, 7 and 8). This type of railing does not meet County or Federal standards and should replaced with MBGR per County standards.

3.3 Culvert Item 62 Condition Rating:

10’-0” x 10’-0” x 45’-9” long RCB culvert could not be observed due to heavy vegetation. Ladder access to channel is necessary to review. No condition rating is assigned.

3.4 Channel Item 61 Condition Rating: 6

O’Leary Creek is a medium to high flow well defined rock and boulder channel. There is pipe and wire on the south embankment and grouted rock on the north upstream embankment. There is pipe and wire on the north bank downstream. The embankments and bottom have heavy vegetation and trees.

3.5 Signing & Markers Item 70 Posting Code: 5

There is Type OM2-1V roadway markers (with bridge number) at the approach ends of the timber railing (Appendix A Photo 1). There are Type OM-3L/R markers at all four corners of the railings (Appendix A Photo 1). There are No load rating postings.

3.6 Utilities There are overhead power lines on the east side of the roadway (Appendix A Photo 1).

ATTACHMENT "A1' . - CONDITION OF STRUCTURE RE PORT

Road Name ail Road County Bridge No. 411 Date of Investigation 2/2/71 CONDITION : There is a large area of exposed aggregate pockets in deck at downstream end. It is partially patched but needs repatching. There are two rock pocket areas approx. 6' to 7' above culvert floor on the south wall. The areas near the bottom of both walls are eroded exposing the aggregate. This is caused by the high velocity flow carrying a heavy rock load during high flows. The bottom is eroded 1/4" to 2" due to high velocity flow and heavy rock load. The wingwalls are chipped all along the edges but no exposed steel. There is some evidence of seepage on the south wall at the top from . . 2 ' to 4' frqm the downstream end. Appears to be at construction joint which has been patched.

RECOMMENDATIONS : Chi out and patch with epoxy mortar all rock pockets greater than 1/2P, depth. .Patch bottom of both walls with epoxy where heavily scoured.

ESTIMATE OF COST: Patch work $600
