HAZARD PREVENTION PROGRAM STANDARD OPERATING PROCEDURE

GENERAL SAFETY MANDATORY REQUIREMENTS 1 Participate in Box Talk to ensure understanding of safety, hazards and procedures and to ask any questions. 2 All workers must wear all required Personal Protective Equipment. 3 Ensure required / effective communication at all times (i.e. radio, hand signals, eye contact, etc.). 4 Identify, eliminate and/or control Environmental hazards. 5 Report immediately any ship deficiencies or equipment that is not operating properly. 6 Watch for moving machinery, gear and equipment. 7 Be aware of all workers in your gang and adjacent operations. 8 Maintain good housekeeping. 9 BE AWARE OF AND STAY OUT OF THE BIGHT. GEAR PERSON/VESSEL AT DOCK 1 Inspect Lift Truck. Being aware of surroundings, dock surface, forks position. Beware of workers in proximity of working equipment. 2 If required, install gangway. Using cradle if required. 3 Position boxes and heads as well as safety cage, position cones/signs on dock when required. Lay out slings. Ensure bull rails are in place, be aware and maintain radio contact with adjacent operations or equipment in area (if required). 4 Ensure the cherry pickers, safety , peaveys, are available to be put on ship. Test run safety saw. 5 Position gear/automatic heads. Be aware of overhead hazards. Beware of workers in proximity of working equipment. 6 Replace damaged slings. Mark/or label damaged slings. Chokers as required on ship. Lifting , (two are required) when lifting catwalk material. Beware of jaggers. 7 When lifting gear (hogwires, boxes, catwalk material, etc.), beware of overhead hazards and keep hands out of the bight. Ensure adequate lifting slings are used. 12 When loading onshore side and wires or chains are being tightened, be aware of operations aboard vessel which may cause falling objects or overhead hazards. Use caution when in the cone zone. 13 As required when setting up to access a deck load, gear person to provide ladder, rope with dog ends, non- slip mat, sledgehammer. Tie off ladder as required. 14 On finishing day ensure all boxes and equipment are in order and deficiencies are reported to Head Foreman.
