o A Macomb County resident o Age 60 or older or be the spouse of a person at least 60 or older. o The person must be unable to attend Dining Senior Style because of physical or emotional disabilities. o The individual must be physically or emotionally unstable to obtain or prepare full meals.

Cost: A $3.00 cost sharing contribution is suggested for the meals.

Eligibility  Services are free to those who qualify.  Must meet current Federal Income Guidelines. Income includes gross income of everyone residing in the home at the time of application.  Recipients of FIP or SI who have received benefits during any of the past twelve months are automatically income eligible.  Applicants must be residents of Macomb County.  Weatherization crews cannot work on any home that is for sale.

Must provide complete proof of income. This could include one or more of the following

Eligibility  Must be a Macomb County resident.  Must be age 60 or older.  Must be income-eligible.

Services Snow Removal

Runs November through March and is performed after snowfall of two or more inches. Under normal conditions, snow is cleared within 48 hours after snowfall has stopped.

Lawn Care

Grass cutting begins in April or May and continues until October.

Cost It is suggested that the senior make an appropriate contribution to share in the cost of providing the service. Your suggested donation is based on your yearly gross income and house size using the cost-share sliding scale shown below. Individual assets, property or savings are not considered when determining the amount.

Macomb Community Action Transport

Eligibility  Must be a Macomb County Resident  Must meet Community Services Block Grant income guidelines  Must complete and application for service at designed Community Action Center  Must have documented need for medical treatments  Does not have access to public transportation (SMART or municipal services)  Does not live with someone who owns and drives a vehicle  A caregiver must assist any rider who is unable to meet the vehicle curbside without assistance

Making a Request  Requests for transportation must be made during regular business hours: Monday – Friday, 8:30 a.m. – 5:00 p.m.  Transportation operates between the hours of 6:00 a.m. – 4:00 p.m. Riders must depart for their final destination by 2:00 p.p.  Requests for rides must be scheduled at least one week in advance of the needed service, two weeks advance notice is suggested.  Requests can be made up to one month in advance  Specify accommodations needed, such as hydraulic wheelchair lift  Must provide the name of the person who will help the rider board and depart the vehicle if assistance in necessary  The day before the scheduled ride, the Transportation program will call to confirm estimated arrival time and the number of people to be transported  Riders should notify the Transportation program if they have arranged for transportation home from a destination  24 hours advance notice is required to cancel a ride  Trip are primarily limited to destination in Macomb county
