A recent prosecution in Queensland has prompted a reminder from Worksafe WA about the potential danger of excavator quick hitches.

Peter Tullett, a mechanical fitter, died from his injuries after being struck by a 1.2t shear attachment on a construction site south of Brisbane in April 2017. Prosecutors alleged that the excavator operator, Neil Edward Norris, had not attached the quick hitch safety pin, resulting in the shear becoming dislodged and striking Mr Tullett in the head. It was further alleged that Mr Norris and his manager then attempted to cover up the incident by reattaching the shear with the safety pin inserted. The tragic incident is the latest in a series of fatalities and serious injuries caused by the failure of quick hitch devices, usually because operators have failed to install the hand-operated safety pin between bucket or attachment changes, or failed to ensure a hydraulically-actuated safety pin is correctly engaged.
