Back to life after 38 days in Ventilator

The story of Netaidas Mukherjee is not only unique, it is also a record of sorts. The 52-year-old social worker from South Kolkata’s Tollygunge got infected by COVID-19 while distributing relief. As he got admitted to AMRI with high fever and breathlessness, even doctors were worried. For 38 days he remained on ventilator, ghting the virus, and came out of it to tell his story. He could not be more grateful to the doctors and clinical team of Dhakuria. Doctors, on the other hand, praise him for being resilient. Mukherjee is the rst such case in India, where a COVID patient stayed on ventilator for so long, and survived.

Battle won after 300 hours on ECMO

The 24-year-old woman from South Kolkata we brought to the Emergency department of AMRI Dhakuria one fateful night in May with high fever and breathing trouble. Within hours she was out on ventilator, and then, with the family’s permission, she was put on ECMO support. The equipment, which re-oxygenates blood, helped balance the oxygen saturation in her blood. After around 300 hours on ECMO, she became the rst COVID patient in India to survive after being put on ECMO support. Earlier, the method was tried at AIIMS Delhi and a private hospital in Chennai but in both cases, the patient succumbed to the disease.

Virtual Visiting Hours

What is ECMO support? Extracorporeal membrane oxygenation, or ECMO, is a machine similar to the heart-lung by-pass machine, often used in open-heart surgeries. The machine infuses oxygen into the blood and removes carbon dioxide, pumping oxygen- ated blood into a patient's blood from the outside, putting the heart and lungs at rest. It is an advanced method of providing life support to people who are seriously ill. While a typical ECMO course runs for around ve days, in some cases, ECMO support has been continued for up to 25-30 days.

What is the role of ECMO in COVID? Some COVID-19 patients get admitted, with very poor oxygen saturation in their blood and they need more than just ventilator support. This is where ECMO comes in play, helping us to stabilise a patient and giving us a fair chance to ght against the virus. We recently used ECMO to treat a 24-year-old patient, whose blood saturation level had gone down much below the desired 90%. Without ECMO it would have been very dicult to bring her back.

What is the outcome of using ECMO support? In COVID-19, respiratory distress is a primary symptom, particularly which often develops into Acute Respiratory Distress Syndrome (ARDS). ARDS has been seen to have a high mortality of over 45% and has remained a challenge for doctors, particularly in the clinical management of COVID-19. A number of doctors have decided to use ECMO support during the COVID pandemic and it is being seen that early use of ECMO has given favourable results and should become a standard protocol in treating ARDS. AMRI has a unique department to nancially support the less privileged patients. Every month about 15-20 less privileged patients are supported for surgery/treatment.
