HS Athletic Information – Fall 2021
1. DIAA Sports Physicals: ● Updated and Current DIAA Physical Forms can be found on: o MHS, AHS, OHS Athletics Websites o DIAA Of icial Website: https://www.doe.k12.de.us/site/Default.aspx?PageID=3300 o Be sure to have the new 21-22 DIAA sports physical
● Physicals for the 2021 school year must be D ATED AFTER APRIL 1, 2021.
● ALL ATHLETES must have a completed and submitted DIAA PHYSICAL FORM to participate.
● It is recommended that all Fall student- athletes submit their completed physical forms AT LEAST TWO WEEKS PRIOR to AUGUST 16, 2021
● Completed DIAA Sports Physical Forms should only be turned into Athletic Director, School Nurse, and or of icial MHS/AHS/OHS School Personnel. PLEASE KEEP A COPY FOR YOUR RECORDS
