We believe these cuts will further strain our health care system, which is already stressed by the COVID-19 pandemic, and jeopardize patient access to medically necessary services over the long-term. On December 1, 2020, the Centers for Medicare & Medicaid Services finalized the Medicare Physician Fee Schedule for 2021. The fee schedule includes several positive attributes, including improvements for maternity care and much-needed payment increases for physicians delivering primary and other essential outpatient and office-based care to some of our nation’s most vulnerable patients. These changes should take effect on January 1, 2021, as planned. However, a statutory budget neutrality rule requires that any increases in Medicare payments for these office visits, also known as evaluation and management (E/M) services, must be offset by corresponding decreases. As a result, many practitioners including surgeons, specialists, therapists and others face substantial cuts beginning on January 1, 2021, if Congress does not take action to provide relief. Health care professionals across the spectrum are reeling from the effects of the COVID-19 emergency as they continue to serve patients during this global pandemic. The payment cuts finalized by CMS would pose a threat to providers and their patients under any circumstances, but during a pandemic the impact is even more profound.

This stopgap solution once again helped provide funding for rural schools, roads, and other critical county services. However, SRS expired at the end of fiscal year 2012, and final payments were distributed to counties earlier this year. If Congress fails to extend or reauthorize the program in a timely manner, layoff notices for rural schoolteachers and road department personnel are a very real possibility. Our members understand the serious budget situation the country is facing and the difficult budget decisions you will be considering in the coming weeks. Nevertheless, the loss of SRS funding will put an enormous strain on already stretched county budgets, limiting local governments in their ability to provide necessary services. Furthermore, counties in California have absorbed billions in state funding cuts and have taken on more and more responsibilities of managing and paying for traditional state-run programs. We want you to fully understand that the loss of SRS funding will be devastating to our members and to rural California.     June 22, 2021 The Honorable Chris Van Hollen The Honorable Cindy Hyde-Smith Chairman Ranking Member Subcommittee on Financial Services Subcommittee on Financial Services and General Government and General Government Committee on Appropriations Committee on Appropriations Dear Chairman Van Hollen and Ranking Member Hyde-Smith: As the Committee considers the FY2022 Financial Services and General Government Appropriations bill, we ask that you include language requiring the Office of Personnel Management (OPM) to address pay disparities between federal wildland firefighters and their counterparts employed by state and local wildfire agencies. Year after year, the annual wildfire season is growing longer and more intense. Last year, the United States experienced one of the most destructive seasons on record with 58,950 wildfires burning more than 10 million acres. In California alone, 9,900 wildfires burned a record-setting 4.25 million acres, killing 33 people and destroying 10,500 structures, more than 5,000 of which were homes. Severe drought conditions and early fire activity this year portend another dangerous fire season ahead. Despite these warning signs, agencies have an inability to hire and retain enough federal wildland firefighters and support fire personnel such as look-outs and fire prevention technicians, primarily due to low pay. Federal wildland firefighters’ salaries are significantly less than their counterparts at state and local wildland fire agencies, even though they have comparable job duties. For example, the starting salary for a Forest Service firefighter in the San Francisco Bay Area— which has a high cost-of-living—is $33,912, compared to a $58,668 starting salary for Cal Fire firefighters.

These figures include six nonvoting Delegates, one each from Guam, Hawaii, the District of Columbia, and American Samoa, and two from the U.S. Virgin Islands, as well as one Resident Commissioner from Puerto Rico. Of these 366 women, there have been 309 (211 Democrats, 98 Republicans) women elected only to the House of Representatives; 41 (25 Democrats, 16 Republicans) women elected or appointed only to the Senate; and 16 (11 Democrats, 5 Republicans) women who have served in both houses. A record 131 women were initially sworn in for the 116th Congress. One female House Member has since resigned, one female Senator was sworn in January 2020, and another female Senator was appointed in 2019 to a temporary term that ended in December 2020.
