Families receiving Early Intervention (EI) services face unique challenges in locating and accessing resources. The “Resource Guide for Families of Children with Disabilities or Developmental Delays: 2016” provides a comprehensive list of community resources for parents and families of young children with developmental delays and disabilities and the professionals who work with them.

The Health Department’s Bureau of Early Intervention created the Social Services Task Force to produce this guide. The bureau would like to recognize the task force members and their agencies for their collaborative efforts in developing this document:

This section includes information on the different types of clinics:

Article 16 Clinics Clinics approved by the New York State Office for People with Developmental Disabilities (OPWDD) to provide long-term therapies (such as physical therapy) to people with developmental disabilities. These services are prescribed by a doctor or other medical professional. Services include helping people with assistive devices such as wheelchairs, hearing aids and walkers.

Article 28 Clinics Clinics that can be located either in a hospital or a non-hospital setting. They are licensed by the New York State Department of Health. Certain Article 28 facilities are designated by the State to serve people with developmental disabilities. NYC Health + Hospitals These are public health hospitals and clinics. They provide families with low-cost and free health care regardless of immigration status or ability to pay. Some offer specific services for families of children with disabilities.

Sensory Gyms provide activities to help children with sensory-processing disorders. Occupational therapists (OT) and physical therapists (PT) may provide services in sensory gyms. Explanation of terms in this section:

Early Intervention (EI) authorization These providers have NYS authorization to provide services to children (birth to three years) who have Individualized Family Service Plans (IFSPs) authorizing OT or PT.

Committee on Preschool Special Education (CPSE) These providers accept children (ages three to five) who have Individual Education Plans (IEPs) authorizing OT or PT.

Related Services Authorization (RSA) For children (ages five to 21) receiving services through the Department of Education. Family Reimbursement Programs Families of children with developmental disabilities can apply to these programs in order to be reimbursed for goods and services that benefit their child with a . There is no guarantee that a family will be granted the requested funds. Amounts granted usually do not exceed $500 per year. Do you know where all of the information on your child is? Can you quickly find copies of the last Individualized Family Service Plan (IFSP)? Progress reports? Immunization records? Prescriptions for services or medications? You have probably discovered that you have a lot of paperwork related to your child’s disability and services. Organizing all of this paperwork when your child is young will help you keep track of his or her progress over time. You can organize the paperwork in one large three- ring binder, in multiple binders by subject, in scanned documents saved in your computer or by any other method that you are comfortable with. The method is less important than the fact that you organize your child’s documents as early as possible.

Here are some ideas on how to organize your files.
