Material and Method: The authors retrospectively reviewed the medical records of patients with RVF treating from 1994 to 2013 at the Faculty of Medicine Siriraj Hospital. Data were recorded including patient’s demographics, etiology, treatment and outcome. Results: This study included 108 patients with median age of 55 years (range 24 to 81). Radiation injury was the most common cause of RVF (44%) followed by direct invasion of rectal or gynecologic malignancies (20%), postoperative complication (16%) (notably from 10 low anterior resections, 5 transabdominal hysterectomies, 1 stapled hemorrhoidopexy and 1 injection sclerotherapy for hemorrhoid) and obstetric injury (11%). Diverting colostomy was the most frequent operation performed for both radiation-related RVF and malignancy-related RVF. Most operation-related RVF were healed after fecal diversion with or without either local repair or major resection. All obstetric-related RVFs were successfully treated by local tissue repair with or without anal sphincteroplasty. Conclusion: Radiation injury and advanced pelvic malignancies were two most common causes of RVF in this study. Fecal diversion can be either initial operation or definite treatment in most patients with RVF. -

Objectives: To evaluate the feasibility, patient acceptance, recur- rence and results of stapled haemorrhoidectomy in our early experience. Methods: Between Jan 2012 and Dec 2013, 42 patients with symptomatic GRADE III and IV hemorrhoids were operated by stapled hemorrhoidectomy by a single surgeon at our surgery department. The evaluation of this technique was done by assessing the feasibility of the surgery; and recording operative time, postoperative pain, complications, hospital stay, return to work and recurrence. Results: All the procedures were completed successfully. The mean (range) operative time was 30 (20 - 45) min. The blood loss was minimal. Mean (range) length of hospitalization for the entire group was 1 (1 - 3) days. Only 3 patients required more than 1 injection of diclofenac (75 mg) while as rest of the patients were quite happy switching over to oral diclofenac (50 mg) just after a single parenteral dose. -

Abstract Introduction: Procedure for prolapsed hemorrhoids (PPH) or hemorrhoidopexy is not free from complications, some of which have been described as serious, such as bleeding. This study describes a case of a female patient with post-operative huge pelvic hematoma, successfully treated with percutaneous angioembolization. Case Presentation: A 76-year-old female underwent PPH, with no intraoperative complications. Few hours later, the patient showed signs of acute abdomen. No external rectal bleeding was identified and vital signs were normal. A computerized tomography (CT)- scan showed a giant peri-rectal and retroperitoneal pelvic hematoma, with signs of active bleeding. A subsequent selective arteri- ography showed huge bleeding from superior hemorrhoidal artery, treated with super-selective embolization. The procedure was successful and the patient showed a symptomatic improvement. The subsequent hospital stay was uneventful and she was dis- charged on the ninth post-operative day, with no complications. At the 30-day post-discharge follow-up, the patient was completely pain free with no signs of pelvic discomfort. -

À ABSTRACT Hemorrhoids are a normal component of the anorectum and contribute to the mechanism of anal closure, thus providing fine adjustment of anal continence. There are numerous myths and legends associated with the disordered and diseased state of hemorrhoids. Fortunately, information obtained from modern technolo- gies including microscopic histopathology defined first the actual substance and makeup of hemorrhoids and was later combined with anorectal physiology to provide evidence establishing the underlying pathophysiol- ogy of this universal finding of the human anorectum. The sliding anal canal theory of Gass and Adams has held up and is further supported by other anatomic studies including the work of WHF Thomson, who popu- larized the term “cushions” to describe the complex intertwining of muscle, connective tissue, veins, arteries, and arteriovenous communications which constitute hemorrhoids. A loss of muscle mass in favor of connec- tive tissue over time helps explain the role of aging as a predisposing factor for symptomatic hemorrhoids. Other factors include the modern “rich” or low-residue diet leading to constipation and straining which con- tributes to prolapsing cushions. Pathologic studies also demonstrated arteriovenous communications explain- ing why hemorrhoid bleeding is typically bright red or arterial in nature as opposed to dark or venous bleeding. -

Nonsurgical pneumoperitoneum has a wide variety of causes, but a transmural anal fissure being the cause has yet to be documented.
