Completion of Work Report (C RW): Removal of chemicals, treatment sludge and the Connex trailer located at 15 Broad Street, Hudson, Massachusetts. The scope of work (SOW) has been completed pursuant to the consent order between H. LaRosee & Sons and the US EPA.

1. In August 2012 the US EPA site coordinator, Ted Bazenas, David LaRosee a representative of the GEL Realty, property owner, and H.LaRosee & Sons Inc. occupant, as well as Matt Badiali, a representative of United Industrial Services (a!k/a Tradebe) reviewed the site of the storage trailer and waste sludge storage drums. At that time the date of September 12, 2012 was mutually agreed upon as the date for the removal of sixteen 55 gallon drums containing depleted chromium solutions (3), depleted nickel solutions (8), solvents (1 ), and wastewater treatment sludge ( 4).

2. To facilitate removal of chemicals from the Connex trailer, John Gouldsbrough was contracted to remove vegetation and debris in the vicinity of the trailer. The removal costs were as follows: Gouldsborough $5190.50 H. LaRosee & Sons employees $6000 (approximate overtime) Total cost: $11,190.50

3. In conjunction with the removal of chemicals from the trailer, and with the consent of the EPA, H. LaRosee & Sons undertook the responsibility of cleaning out the wastewater pH neutralizing system. The sludge from the three tanks was also designated for removal by United Industrial Services. The costs associated for cleaning the wastewater system were approximately $3840

4. In preparation for the removal of drums they were sampled by the representative of United Industrial Services, those samples were analyzed by United and subsequent profiles were created for each drum on May 16,2012. A listing of the drums is attached to this report. Any jars, pails buckets etc. or other containers within the storage trailer had previously been removed with the acquiescence of the EPA. The costs associated with removal items from the trailer was approximately $2880 (employee overtime)

5. As previously noted the contents of the 55 gallon drums had been identified by United as follows: 3 drums of depleted hexavalent chromium plating solutions 8 drums of depleted nickel plating solutions 1 drum containing solvents 4 drums containing wastewater treatment sludge

6. United categorized and staged containers holding contents identified to be hazardous substances or pollutants or contaminants as required by the order in preparation for removal.

7. On September 12, 2012 a representative from United picked up the drums for off-site disposal. Manifests are attached to this report. The EPA site coordinator was present and examined the manifests placards etc. The cost of associate with items 3, 4, 5, and 6 was 5877.31.

8. On September 15, 2012 the Connex trailer was removed from the site. The reported cost for removal of the trailer was approximately $4000. Costs associated with the trailer removal $4,000 (paid by others)

9. On October 19, 2012> after discussions with the EPA, Jonathon Moore a representative of Ornni Environmental Services conducted soil sampling for analysis of soils under and around the storage trailer site. The soils were analyzed and a report generated was on November 2, 2012 indicating" reportable contaminants were present". Omni is required to report these findings to the Massachusetts DEP and will do so. Costs associated with Omni Environmental were $4122.70.

10. Analytical results obtained by Omni indicate that excavation and staging of contaminated soils is not currently necessary. Alternatives such as capping with asphalt or packing gravel on top of the purported contaminants are being explored. The findings by Omni were consistent with findings by the EPA when it analyzed the area two years ago. It is deduced that the small area where lead was discovered was contaminated from the improper disposal of batteries. The concentrations of cadmium and chromium are more probably than not the result of the leather operations of the original shoe factory that operated on the site (Braga Shoe). 11. Any necessary local state or federal permits for disposal, handling and storage of hazardous waste, hazardous substances, pollutants or contaminants were obtained United Industrial Services. There were no response related repairs to be made.

12. No other specific actions related to the removal of chemistry have been requested by the OSC.

Total Cost of Project: $31,910.51

On October 19, 2012, Omni Environmental Group collected soil samples to characterize surficial soil conditions associated with the storage trailer formerly located in the southwest corner of 15 Broad Street in Hudson, Massachusetts.

Sample Collection

Omni Environmental Group installed soil borings at 5 locations within the footprint of the former storage trailer and 6 locations around the perimeter of the former storage trailer. We show the location of the former storage trailer and the 11 borings (B-1 through B-11) on the Site Plan.

Omni Environmental Group installed borings B--11 through B-4 approximately 2 feet inside each end of the former trailer and 2 feet inside each side of the former trailer. We installed boring B-5 in the approximate center of the former trailer footprint. We installed borings B-6 through B-11 approximately 2 feet outside of the former trailer footprint.

Omni Environmental Group cleared organic material (leaves and woody material) from each location prior to installing the boring. We installed the borings to a depth of 18 inches below grade using handheld, direct push sampling tools advanced with a rotary hammer. During advancement of the borings weDRAFTcollected soil samples on a continuous basis. Soil observed by Omni Environmental Group generally consisted of fine to medium sand with gravel, coal pieces, ash and slag. An organic topsoil was observed at the surface in several borings.

Sample Analyses

Omni Environmental Group screened the soil samples with a photoionization detector (PID) with a 10.6 eV lamp and a headspace screening method. The PID was calibrated to measure volatile organic compounds (VOCs) in parts per million (ppm), benzene referred. VOC concentrations were less than 1 ppm except in borings B-1 and B-11 which contained VOC concentrations of 4.5 and 1.4 ppm, respectively. Omni Environmental Group collected discrete soil samples from borings B-1 through B-5 (sample ID B-1 through B-5) for laboratory analysis. Each discrete sample contained soil across the entire depth of the boring. We composited soil samples collected from borings B-6, B-7 and B-8 into one sample (sample ID CS-1). We composited soil samples collected from borings B-9, B-10 and B-11 into one sample (sample ID CS-2).

Omni Environmental Group submitted these samples to Con-Test Analytical Laboratory of East Longmeadow, Massachusetts for analysis. Con-Test analyzed samples B-1 through B-5, CS-1 and CS-2 for: volatile organic compounds by USEPA Method 8260B; RCRA 8 metals by USEPA Methods 6010B/7471A; oxidation reduction potential by Standard Method 2580A and pH. Con-Test also analyzed sample CS-1 for polychlorinated biphenyls by USEPA Method 8082.

Omni Environmental Group has summarized the data for detected compounds in Table 1 (VOCs) and Table 2 (metals). We’ve attached a copy of the analytical report.

Data Evaluation

As requested by the USEPA, Omni Environmental Group compared the analytical data against the Massachusetts Department of Environmental Protection’s (MassDEP’s) Reportable Concentrations and Method 1 Risk Characterization Standards from the Massachusetts Contingency Plan (MCP). The MCP is the MassDEP’s regulation regarding the assessment and cleanup of releases of oil and hazardous materials.

The Reportable Concentrations are concentrations which, if exceeded, require additional actions to be performed under the MCP. The Method 1 standards evaluate the potential risk of oil or hazardous materials to human health and the environment. Remediation of the oil or hazardous materials may be required if the Method 1 standards are exceeded. Omni Environmental Group shows both the Reportable Concentrations and the Method 1 Risk Characterization Standards on the data summary tables.

The concentrations of the following compounds exceeded their associated Reportable Concentrations in one or more samples:

• Trichloroethylene DRAFT • Cadmium • Total chromium • Lead

The Method 1 Risk Characterization standards are based on either the current manufacturing use of the property (the S-2/GW-2 and S-2/GW-3 standards) or potential future residential use of the property (the S-1/GW-2 and S-1/GW-3 standards). The concentrations of one or more of the following compounds exceeded their associated Method 1 soil standards:

• Trichloroethylene (both S-1 and S-2) • Cadmium (S-1 only)

Future Actions

Based on these assessment activities, Omni Environmental Group suggests the following:

• Analyze all 7 soil samples for hexavalent chromium. If the total chromium is determined to be trivalent chromium and not hexavalent chromium, the applicable Reportable Concentration and Method 1 Risk Characterization Standards will be significantly higher.

• Seek clarification from the USEPA and the MassDEP regarding the applicability of the MassDEP requirements under the MCP, including the notification requirements. Although the USEPA has requested a comparison of the analytical data to the MCP standards, it is our understanding that the requirements of the MCP do not apply to contamination associated with the former storage trailer.

• Request information from the USEPA regarding: 1) additional actions they may require to further evaluate contamination in this area; 2) applicable contaminant action levels and cleanup levels; and 3) response action objectives and potential endpoints.
