1. An acknowledgement that the institution signed and returned to the Department the Certification and Agreement and the assurance that the institution has used no greater than 50 percent of the funds received under Section 314(a)(1) of the CRRSAA to provide Emergency Financial Aid Grants to students.

Yes, Twin City Beauty College (main campus) and our (3) branch locations, Traverse City Beauty College, Michiana Beauty College, and Tulip City Beauty College have distributed no greater than 50%, within the first quarter, of the funds to provide Emergency Financial Aid Grant to students.

2. The total amount of funds that the institution will receive, or has received from the Department under the HEERF II student portion.

Total amount of fund: $189,422.00

3. The total amount of Emergency Financial Aid Grants distributed to students under Section 314 (a)(1) of the CRRSAA Act as of the date of submission.

Total amount distributed to students as of May 17, 2021: $94,707.00

4. The estimated total number of students at the institution eligible to participate in programs under Section 484 in Title IV of the Higher Education Act of 1965 and thus eligible to receive Emergency Financial Aid Grants to students under Section 314(a)(1) of the CRRSAA:

5. The total number of students who have received an Emergency Financial Aid Grant to students under Section 314(a)(1) of the CRRSAA as of May 17, 2021:

6. The method used by the institution to determine which students receive Emergency Financial Aid Grants and how much they would receive under Section 314 (a)(1) of the CRRSAA.

Twin City Beauty College and all branch locations evenly divided the funds to be given amongst 102 eligible students.

7. Any instructions, directions, or guidance provided by the institution to students concerning the Emergency Financial Aid Grants.

Twin City Beauty College and all branch locations required students to sign our ACH REQUEST, HIGHER EDUCATION EMERGENCY RELIEF FUND – HEERF II STUDENT ACKNOWLEDGEMENT FORM which states the use of the emergency grant aid “may be used for any component of my cost of attendance, or emergency costs that arise due to the coronavirus, such as tuition, food, housing, health care (including mental health care) or child care.” This ACH Request allowed us to directly deposit the aid into the student’s checking or savings account within the 15 calendar day obligation from the date of drawing down funds.
