Data extraction date

All data in this report is based on appointment dates up to the end of the last day in each month. Data revision Minor variations for totals published in previous reports are likely to occur as the outpatient databases are continuously updated. Variations may occur for a number of reasons, such as updates to procedure codes, lags in data processing or other types of errors detected after publication.

There is ongoing revision of the data from January 2013 to the current month. Therefore, the most recent edition of a report should always be consulted, as data is not revised in previously published editions.

As part of the reconfiguration of the South Metropolitan Health Service, patients have been transferred between hospitals or have been discharged. Outpatient cases have also been reviewed as part of administrative and clinical processes.

Data source

The data sources are TOPAS (The Open Patient Administration System) and webPAS (web- based Patient Administration System). Data from these patient administration systems are extracted into the Non-Admitted Patient Activity and Wait List Data Collection (NAPAAWL DC), which is a central reporting data collection.

Data limitations

As of the July 2014 report, all outpatient data except ‘Did not attend’ (DNA) appointments are reported as Service Events to align this report with national definitions. DNA appointments are still reported as Occasions of Service as they are considered non-service events, and hence they cannot be compared to other outpatient activity figures.

The main differences between Occasions of Service and Service Events are that Service Events include only clinical activity and a single event within the one day and are auditable, whereas Occasions of Service may include non-clinical activity and possible multiple events within the one day and are not auditable. These differences result in the number of Service Events always being lower than the number of Occasions of Service, that is, the number of Service Events is a subset of the number of Occasions of Service.

A new patient administration system (webPAS) was implemented at Fremantle Hospital on 23 May 2012, Swan District Hospital on 4 July 2012 and Kalamunda Hospital on 4 July 2012 to replace TOPAS. As a result of this implementation, some specialty codes have been modified resulting in specialty data for these hospitals not being comparable against prior periods and across hospitals.

Royal Perth Hospital Shenton Park Campus (RPRH) was closed in early October 2014. Kaleeya Hospital was closed in early November 2014. Fiona Stanley Hospital (FSH) opened in early October 2014. Initial monthly wait time data from October 2014 to January 2015 for Fiona Stanley Hospital is affected by a small count number from specialties with high wait times. Increasing counts and establishment of normal clinic operations has normalised the wait times from February 2015.

Data exclusion

Counts are based on selected specialties for both teaching and non-teaching hospitals. This means, for example, that some types of appointments, such as physiotherapy, are not counted. As the data in this report refers only to medical and surgical outpatient activity, it does not include data related to allied health and diagnostic outpatient services.

Effects of rounding

Where figures have been rounded, discrepancies may occur between sums of the component items and totals. Published percentages are calculated prior to rounding of the figures and therefore some discrepancy may exist between these percentages and those that could be calculated from the rounded figures.

Booked appointment

Due to inconsistencies in the process used for classifying booked appointments for outpatient services, separate data is provided with respect to both first and new appointments.

First time appointment

A first time appointment refers to an appointment where a patient has never before attended a clinic of that specialty at that hospital.

New appointment

New appointments can have multiple entries, as the appointment recording system allocates a maximum of 45 minutes per appointment. If a further extended appointment is required, this attendance is also recorded as a ‘new’ appointment until a follow up session is required.

Follow up appointment

A follow-up appointment refers to an appointment subsequent to and typically shorter than a new appointment e.g. 15 to 20 minutes in duration.

Waiting time

Calculation of waiting time is based on first appointments, in preference to new appointments, as the latter include multiple counts of attendance at an outpatient clinic.

Specialty

The list of outpatient specialties refers to clinically related provision of services, as defined by each hospital.

Definition of appointment

Attendance at an outpatient appointment is based on counts of medical and surgical appointments, as recorded within the NAPAAWL data collection.

Metropolitan public hospitals

The term ‘metropolitan public hospital’ refers to Princess Margaret Hospital for Children and the public hospitals located in the North Metropolitan and South Metropolitan Health Services.

Teaching hospitals

Encompasses adult metropolitan teaching hospitals Fremantle Hospital, Fiona Stanley Hospital, King Edward Memorial Hospital for Women, Royal Perth Hospital Shenton Park Campus (now closed), Royal Perth Hospital Wellington Street Campus, Sir Charles Gairdner Hospital and the metropolitan children’s hospital (Princess Margaret Hospital for Children).

Non-teaching hospitals

Encompasses the following metropolitan hospitals: Armadale-Kelmscott Memorial Hospital, Bentley Hospital, Kalamunda Hospital, Osborne Park Hospital, Rockingham General Hospital and Swan Hospital District Hospital.

Glossary

Mean: This refers to the arithmetic mean, being the sum of all values in the list divided by the number of items in the list. The mean is the most commonly used type of average and can also be referred to simply as the average.

Median: This refers to the value separating the higher half of a sample, a population, or a probability distribution, from the lower half. This statistic is used, in preference to the mean, when the distribution of the values is skewed by a small number of very high values.

90th percentile: This refers to the value in the list below which there are 90 percent of numbers and enables waiting times to be expressed as a percentile rank.

As percentile ranks are not on equal interval scales, the difference between two percentile scores is not necessarily the same as the difference between two other similarly separated percentiles. e.g. 90th percentile score subtract 40th percentile score does not necessarily equal 80th percentile score subtract 30th percentile score. 1. Executive summary

The purpose of this report is to provide summary information about attendances at outpatient clinics operating at metropolitan teaching hospitals, including Fremantle Hospital (FH), Fiona Stanley Hospital (FSH), King Edward Memorial Hospital for Women (KEMH), Princess Margaret Hospital for Children (PMH), Royal Perth Hospital Shenton Park Campus (RPRH – now closed), Royal Perth Hospital Wellington Street Campus (RPH) and Sir Charles Gairdner Hospital (SCGH).

In addition to activity at metropolitan teaching hospitals, which accounts for the greatest proportion of outpatient clinics, information is also presented for outpatient clinics at metropolitan non-teaching hospitals.

1.1 Key points

June 2014 to June 2015

Appointments At metropolitan teaching hospitals:

 all (new and follow up) appointments, decreased by 1,728 (2.7%), from 63,177 to 61,449

 new appointments increased by 1,323 (9.0%), from 14,636 to 15,959

 follow up appointments decreased by 3,051 (6.3%), from 48,541 to 45,490

 first time appointments increased by 937 (7.4%), from 12,612 to 13,549

 DNA appointments decreased by 550 (6.4%), from 8,629 to 8,079

Waiting times to first appointment

At metropolitan teaching hospitals:

 median waiting time increased from 0.92 to 0.95 months - an increase of 3.6%

 waiting time at the 90th percentile increased from 7.40 to 9.83 months - an increase of 32.9%.

2. Metropolitan teaching hospitals

This section provides details of outpatient appointments and waiting times at metropolitan teaching hospitals. It compares the current reporting month to the previous month, and to the same month last year. Time series data from January 2013 up to the current reporting month is provided in appendix pages A-1 to A-8.

2.1 Types of appointments

2.1.1 All (new and follow up) appointments

Table 1 (page 2) shows that in June 2015 there were 61,449 all (new and follow up) appointments - a decrease of 857 compared to the previous month (62,306) and 1,728 fewer than the same month last year (63,177). See also Table A-1 (page A-1) for a breakdown of all appointments for each hospital.

2.1.2 New appointments

Table 1 (page 2) shows that in June 2015 there were 15,959 new appointments - an increase of 55 compared to the previous month (15,904) and 1,323 more than the same month last year (14,636).

2.1.3 Follow up appointments Table 1 (page 2) shows that in June 2015 there were 45,490 follow up appointments - a decrease of 912 compared to the previous month (46,402) and 3,051 fewer than the same month last year (48,541).

2.1.4 Did not attend appointments11

There were 8,079 DNA appointments in June 2015 - an increase of 656 compared to the previous month (7,423) and 550 fewer than the same month last year (8,629). 2.1.5 First time appointments

Table 2 (page 5) shows that in June 2015 there were 13,549 first time appointments - an increase of 122 compared to the previous month (13,427) and 937 more than the same month last year (12,612). See also Table A-6 (page A-6) for a breakdown of first time appointments for each hospital. Figure 4 (page 5) provides an overview of first time appointments over the past two years.

2.2 Waiting times

Table 2 (page 5) provides data on the median and 90th percentile waiting times to first appointment at metropolitan teaching hospitals. Figure 4 (page 5) shows the median waiting time to first appointment over the past two years.

This figure shows a steady trend over the two year period up to the current reporting month. See Tables A-7 (page A-7) and A-8 (page A-8) for a breakdown of waiting times to first appointment for each hospital.

3. Metropolitan non-teaching hospitals

This section provides outpatient clinic data from metropolitan non-teaching hospitals. This data includes all service events at these hospitals that may or may not match the specialty services at metropolitan teaching hospital outpatient clinics.

There were 8,954 new appointments at metropolitan non-teaching hospitals for June 2015 – a decrease of 5 compared to the previous month (8,959) and 1,000 more than the same time last year (7,954). See Table 3 (page 7). There were 27,557 total appointments at metropolitan non- teaching hospitals for June 2015 – a decrease of 255 compared to the previous month (27,812) and 480 more than the same time last year (27,077). See Table 3 (page 7) for the data breakdown since January 2013. Figure 5 (Page 6) provides an overview of new and all appointments at metropolitan non-teaching hospitals over the past two years.

4. Individual hospitals This section provides a breakdown of outpatient appointments at FH, FSH, KEMH, PMH, RPH and SCGH over the past two years. It should be noted that some of the variation between hospitals is related to demographics and casemix.

4.1 Appointments

4.1.1 All appointments Figures 6 and 7 (page 8) provide an overview of trends in all appointments over the past two years. See also Table A-1 (page A-1) for a breakdown of all appointments at each hospital.

4.1.2 New appointments Figures 8 and 9 (page 9) provide an overview of trends in the proportion of new appointments (of all appointments) over the past two years.

These figures show that the trend for RPH, FH and PMH is increasing, the trend for SCGH is steady while the trend for KEMH is decreasing. See also appendix Table A-2 (page A-2) and Table A-5 (page A-5) for a breakdown of the number and proportion of new appointments for each hospital. Trends for FSH are yet to be established.

4.1.3 Follow up appointments Figures 10 and 11 (page 10) provide an overview of trends in the proportion of follow up appointments (of all appointments) over the past two years.

The trend for RPH, FH and PMH is decreasing, the trend for SCGH is steady and the trend for KEMH is increasing. See also Table A-3 (page A-3) for a breakdown of follow up appointments at each hospital. Trends for FSH are yet to be established.
