Dear Friends, For over 70 years, Cardinal McCloskey Community Services has strived to protect, empower and promote independence for at-risk children and families as well as those with developmental disabilities through an array of community based services. On a continuous basis, we monitor the needs of those we serve to optimize their care. In 2018, we were successful in our effort to grow our organization to meet these needs. Education initiatives, both within our Early Childhood Education Division (ECED) and our Strategies for Success program for children in foster care also continued to grow allowing us to launch advocacy and enrich educational services at an earlier age to magnify the impact of these efforts. We have expanded our ECED programs through new Universal Pre-Kindergarten (UPK) classrooms for three year-olds and added Early Learn centers, bolstering CMCS’ role as the largest Early Childhood Education provider in . Our Children’s Services program secured new funding to expand Strategies for Success; a comprehensive program providing guidance and individualized supports for youngsters in foster care. The guidance and support, as well as tutoring and advocacy, are all invaluable services that are making a tremendous impact on the lives of these children. For our Developmental Disabilities Services program, 2018 was a busy year, as we secured approval and funding for two new homes, including one for individuals who are currently living at home with their families, and a second for those aging out of residential schools. In addition, we received funding for our ABA (Applied Behavior Analysis) program for adults with autism, thus creating the opportunity for growth through formal training and certification of the clinical staff serving this community. Perhaps one of the most exciting projects of the year was planning for the Cardinal McCloskey Community Charter School to open in September 2019. The charter school will partner with CMCS to help provide students and their families with a variety of wrap-around services. The academic program will provide students with a rigorous, standards-based education in a nurturing, supportive environment. The school will play a critical role in helping at-risk children excel, while laying the foundation for them to live safe, healthy and independent lives. These achievements could not have been possible without our dedicated and gifted staff whom we strive to provide the professional and leadership opportunities that are critical to their own growth and our agency’s success. With the help of our committed donors, we will continue to meet the needs of the communities we serve through all of our CMCS programs. The dedicated support of our donors furthers our work in caring for all the children, families, and adults with developmental disabilities who we are proud to call our CMCS family. Sincerely,

CONTINUED FROM PAGE 5 Family Treatment and Rehabilitation Preventive Services are intended to address the safety and wellbeing of children in high-risk families when a caretaker or child’s substance use or mental health disorder places the child at risk of foster care placement or replacement. The model calls for treatment to be provided in three phases: Initial, Baseline and Stabilization. The service plan is guided by comprehensive assessments and a multi-disciplinary approach involving different interventions in order to meet the needs of the families that are being served. The FT/R utilizes a team approach to assess, diagnose and treat each family member’s needs.

The Family Connections Program provides community based, in-home services to families with children who reside in the East Harlem neighborhood of , and works closely with parents to develop plans that will promote the achievement of their own family’s goals. This revolutionary program offers 14 additional services in these key areas aimed at improving children’s quality of life until they are 21 years old. Our B2H program service delivery is designed specifically for these vulnerable children and their caregivers to promote permanency and maximize functioning in their home, community, and school settings. A unique feature of this program is that services can continue for children and families after adoption, a time when significant support is needed to ensure that a child has successfully adapted to their permanent environment. CMCS operates one of the largest B2H programs in New York City.•

This past year, we responded to changes in the field as the state prepares for managed care, strengthens its service planning, and pursues a value-based payment structure. Two specific changes included the conversion of our St. Theresa’s ICF (Intermediate Care Facility) to an IRA (Individualized Residential Alternative), joining our 17 other group homes classified as IRAs. This change is the result of a federal mandate which set new standards for home and community-based settings and person- centered planning. CMCS’ service planning practice was validated in that it had already met most of the new standards. The second change was the transfer of MSC (Medicaid Service Coordination) to Care Coordination Organizations (CCOs), with the first of a two-step process to further separate this service from others to support an unbiased approach. CMCS successfully transferred this service effective July 1, 2018 and in so doing announced its affiliation with Advance Care Alliance (ACA) as its CCO partner.
