XLIV Liability Waiver Packet VOLUNTEER WAIVER, RELEASE OF LIABILITY & COVENANT NOT TO SUE BRIEF DESCRIPTION OF EVENTS (“EVENTS”): NFL PRO BOWL PREGAME AND HALFTIME SHOW, AND SUPER BOWL XLIV PREGAME, HALFTIME, AND POSTGAME SHOW REHEARSALS AND PERFORMANCES AS SCHEDULED 1. I attest and verify that I am free from all illnesses, injuries or defects and that I am physically fit and sufficiently trained to participate in all activities associated with the Events. My participation in activities and events organized or sponsored by the NFL is voluntary. 2. I am aware that there are risks to my personal safety and/or property, including the risk of serious injury, damage or loss of property, and/or death in conjunction with my participation in activities and events, and I assume all risks associated with my participation in activities and events organized or sponsored by the NFL or connected with the Events. 3. I do hereby, for myself, my heirs, administrators, executors, guardians, assigns, representatives, and next of kin, forever WAIVE, RELEASE, DISCHARGE, AND COVENANT NOT TO SUE the National Football League, its member professional football clubs, NFL Ventures, L. I consent to administration of first aid and other medical treatment in the event of injury or illness and hereby release and indemnify Releasees from any and all liability or claims arising out of such treatment. 5. I hereby grant full permission to Releasees, as described above to use any photographs, images, videotapes, motion pictures, recordings, or any other record of the activities of the Events for any legitimate purpose and I understand that I shall not be entitled to any compensation therefore.
