Before beginning this polygraph examination, I am required, pursuant to the Standards of Practice established by the Departnent of Professional and Occupational Regulation, to explain i" ttriting provisions 3.1 through 3.8, of the regulations governing polygraph. Upon your uriüen reques! an audio-tape record of this examination will be made. The recordingwill be kept with other records required by the Departnent of Professionál and Occupational Regulation and upon request a copy will be made available ûo the Deparünent of Professional and Occupational Regulation, to you or your afiorney. A fee of $25.00 will be charged to you for furnishing a copy of this tape recording. (3.2)

You may request a copy of all portions of any report pertaining to this examination which was prepared by the examiner and provided to any pen¡on. In the event you wish a copy of such report, you must make your request in vyriting:ûo me. The report will be provided to you within l0 business days of your request, or of the date the report is provided to any person, whichever may occur larcr.
