Hyperthermia, defined as a core temperature of N40.5 °C, may present with sweating, flushing, tachycardia, fatigue, lightheadedness, headache, and paresthesia, progressing to weak- ness, muscle cramps, oliguria, nausea, agitation, hypotension, syncope, confusion, delirium, seizures, and coma. Mental status changes and core temperature distinguish potentially fatal heat stroke from heat exhaustion. Man- agement requires the immediate reduction of core temperature. Ice water immersion has been shown to be su- perior to alternative cooling measures. Avoidance of thermal risk and early recognition of cold or heat stress are the cornerstones of preventive therapy. © 2016 The Author. Published by Elsevier B.V.     REVIEWS Recovery from disorders of consciousness: mechanisms, prognosis and emerging therapies Brian L. Edlow 1,2, Jan Claassen3, Nicholas D. Schiff4 and David M. Greer 5 ✉ Abstract | Substantial progress has been made over the past two decades in detecting, predicting and promoting recovery of consciousness in patients with disorders of consciousness (DoC) caused by severe brain injuries. Advanced neuroimaging and electrophysiological techniques have revealed new insights into the biological mechanisms underlying recovery of consciousness and have enabled the identification of preserved brain networks in patients who seem unresponsive, thus raising hope for more accurate diagnosis and prognosis. Emerging evidence suggests that covert consciousness, or cognitive motor dissociation (CMD), is present in up to 15–20% of patients with DoC and that detection of CMD in the intensive care unit can predict functional recovery at 1 year post injury. Although fundamental questions remain about which patients with DoC have the potential for recovery, novel pharmacological and electrophysiological therapies have shown the potential to reactivate injured neural networks and promote re-emergence of consciousness. In this Review, we focus on mechanisms of recovery from DoC in the acute and subacute-to-chronic stages, and we discuss recent progress in detecting and predicting recovery of consciousness. We also describe the developments in pharmacological and electro- physiological therapies that are creating new opportunities to improve the lives of patients with DoC. There are specific neurotransmitters in each system that influence ganglionic and post-ganglionic function (fig 2). The symptoms and signs of autonomic disease cover a wide spectrum (table 1) that vary depending upon the aetiology (tables 2 and 3). In some they are localised (table 4). Autonomic dis- ease can result in underactivity or overactivity. Sympathetic adrenergic failure causes orthostatic (postural) hypotension and in the male ejaculatory failure, while sympathetic cholinergic failure results in anhidrosis; parasympathetic failure causes dilated pupils, a fixed heart rate, a sluggish urinary bladder, an atonic large bowel and, in the male, erectile failure. With autonomic hyperac- tivity, the reverse occurs. In some disorders, particularly in neurally mediated syncope, there may be a combination of effects, with bradycardia caused by parasympathetic activity and hypotension resulting from withdrawal of sympathetic activity. The history is of particular importance in the consideration and recognition of autonomic disease, and in separating dysfunction that may result from non-autonomic disorders. It is mainly idiopathic but it can be associated with severe diseases. Herein, we report two patients with HS. Observation: Observation N°1: A 27-year-old man with a history of migraine, consulted us for a unilateral erythrosis on his face. He reported a redness associated with a hyperhidrosis with strictly medial limits at the left side. It contrasted with anhidrosis and a normal appearance of controlateral hemiface. These attacks were triggered by exercise. They spontaneously disappeared at rest. The clinical examination especially neurological assessment showed no abnormality. Observation N°2: A 22-year-old man with no special medical history. He complained about same symptoms as the first patient but the attacks occurred without any triggering factor. The neurological examination revealed an incomplete Horner’s syndrome (eyelid ptosis, enophtalmos and a tendency to miosis). The two patients were diagnosed with HS. In the two cases, the etiological investigation was normal. Key message: HS corresponds to a unilateral dysfunction of sympathetic system characterized by hypohidrosis and loss of facial erythrosis to heat, exercise, or emotional factors. This phenomenon is compensated by excessive controlateral sweating and redness. It may be isolated or integrated into other dysautonomic syndromes like Horner’s syndrome.

It is a form of hair shaft disorder with increased fragility. The condition is classified into inherited and acquired. Inherited forms may be either isolated or associated with numerous genetic diseases or syndromes (e.g., Menkes disease, Björnstad syndrome, Netherton syndrome, and Bazex-Dupré-Christol syndrome). Moreover, pili torti may be a feature of various ectodermal dysplasias (such as Rapp-Hodgkin syndrome and Ankyloblepharon-ectodermal defects-cleft lip/palate syndrome). Like Horner syndrome, associating miosis, ptosis, and anhidrosis, Harlequin syndrome is caused by disruption of the cervical sympathetic pathways. Authors of this report describe the case of a 55-year-old female who presented with both Harlequin sign and Horner syndrome immediately after anterior cervical discectomy (C6–7) with cage fusion and anterior spondylodesis. They discuss the pathophysiology underlying this striking phenomenon and the benign course of this condition.
