Title Nurse Driven Foley Removal Protocol

Requestor Leadership Application PowerChart Impacted Positions Nursing, Providers PROD Date August 15, 2018 Request Goal Allow nursing the opportunity to D/C Foley for selected patient types without a provider’s order. Per the protocol, nursing SHOULD NOT D/C any Foley without a provider’s order for any of the following: SIADH, Diabetes Insipidus (DI), Cerebral Consult Salt Wasting-ICU Hourly Urinary Output-Foley Only Urology-Placed Foley Catheter Means Acute /Obstruction Active Epidural Use Specific Surgical Procedures: Incontinent: / Urologic Surgery OB/GYN Surgery Low Pelvic/Rectal General Surgery (<3days post op) Renal Transplant Surgery Hematuria Neurogenic Bladder with Chronic Foley

Details • Providers will continue to get CAUTI alerts to document continued Foley reason if upon order entry, an indication was selected that requires a provider’s order to DC the Foley. • Nursing will document the continued Foley reason for Foley if an indication that does not require a provider’s order to D/C Foley was selected upon order entry, so providers will not get CAUTI alerts for these indications. • The Post Foley Catheter Removal Protocol PowerPlan orders should be ordered per protocol by nursing when a Foley is removed per the protocol. Per Protocol orders route to the provider’s message center for signature. • PLEASE NOTE: Modifications have been made to the Foley Urinary Catheter (Catheter Urinary Indwelling) order for this change. PLEASE review the Catheter Urinary Indwelling order within any powerplan favorites where

the Catheter Urinary Indwelling order was preselected as a favorite, make any desired changes and RESAVE the favorite. Re-saving powerplan favorites IS REQUIRED if an indication was saved as a favorite that has been modified or deleted in the order.

Resaving Powerplan Favorite: • A prompt will appear when accessing a saved favorite plan, notifying the provider a change has been made to the plan.

• Select Continue to load new plan then OK, recheck Catheter Urinary Indwelling order if desired, select new reason/indication and other appropriate details, then select Save as My Favorite.
